Gleason 7 skoru orta derecede diferansiye bir tümör kabul edilmeli mi?

Özet
Prostat kanserinin diferansiyasyonunu belirlemede en çok kullanılan Gleason skorlama sistemindeki 5-7 skorları geçmişte orta derecede diferansiye kabul edilirken Gleason 7 skorunun daha agresif davranış gösterdiği ve bu skorun ayrı bir sınıflamaya sokulması gerektiği ileri sürülmüştür. Gleason 7 skorunun özellikleri 80 prostat kanserli hastada evre ve PSA ilişkisi temelinde incelendi ve diğer skor grupları ile karşılaştırıldı. Bütün olgularda tanı transrektal iğne biyopsisi ile kondu, klinik evrelendirme yapıldı ve tedavi öncesi PSA değerleri elde edildi. Klinik evrelendirmede Tl ve T2 kabul edilen olgularda radikal prostatektomi sonrasında patolojik evrelendirme yapıldı ve son Gleason skorları belirlendi. Hastaların yaşı 51 ile 78 arasında değişmektedir (ortalama, 66.42±9.93 yıl), Gleason 5-6, Gleason 7, Gleason 8-10 olgu gruplarının yaşları arasında anlamlı fark yoktur (p> 0.05). Aynı gruplar-da, tedavi öncesi ortalama PSA değerleri, sırasıyla, 9.81 ±5.85; 17.42±7.93 ve 57.23±70.28 ng/ml olarak belirlenmiş-tir. Gleason 7 skorlu olgulardaki PSA değerleri, Gleason 5-6 (p= 0.0005) ve Gleason 8-10 (p= 0.022) skorlu olgulardan anlamlı olarak farklıdır. Patolojik evrelendirmede, Gleason 5-6 skorlu olguların % 7.69'unda tümörün prostat dışına yayıldığı belirlenirken, Gleason 7'de % 40.91, Gleason 8-10'da % 93.75 oranında T3 veya metastatik prostat kanseri saptanmıştır. Ekstraprostatik hastalık varlığı yönünden Gleason 7 skoru hem Gleason 5-6 (p= 0.00637), hem de Gleason 8-10 (p= 0.00002) grubundan farklıdır. Gleason 7 skorlu prostat kanseri hem Gleason 5-6, hem de 8-10 skorlu tümörlerden tedavi öncesi PSA değerleri ve evre açısından farklı görünmektedir ve ayrı bir sınıflamada yer alması gerektiği düşünülmüştür.
Abstract
In the Gleason scoring system which is the most common grading system used in prostatic carcinoma, scores 5 to 7 were previously classified as moderately differentiated tumors. However, Gleason 7 tumors were found to display a more aggressive biologic behavior and suggestions for a separate class for score 7 were made. The features of Gleason 7 score in terms of stage and PSA level were evaluated in 80 patients with prostate cancer in comparison to other score groups. After the diagnosis was made with transrectal needle biopsy, all patients were clinically staged and pretreatment PSA levels were obtained. Patients with clinical Tl and T2 disease underwent radical prostatectomy that made pathological staging and determination affinal Gleason scores possible. The age of the patients ranged between 51 and 78 years (mean; 66.42+9.93) and the groups with Gleason 5-6, Gleason 7, Gleason 8-10 scores were comparable with regard to age (p> 0.05). The mean pretreatment PSA values for the same groups luere 9.81+5.85,17.42+7.93 and 57.23+70.28 ng/ml, respectively. The PSA levels of patients in Gleason 7 group were found to be significantly different than those of patients in Gleason 5-6 (p= 0.0005) and Gleason 8-10 (p= 0.022) groups. Pathological staging revealed that extraprostatic disease was present in 7.69 % of patients with Gleason scores 5-6, whereas stage T3 or metastatic tumor was present in 40.91 % of Gleason score 7 ana 93.75 % of Gleason 8-10 tumors. With respect to extraprostatic extention Gleason 7 score was significantly different than both Gleason 5-6 scores (p= 0.00637) and Gleason 8-10 scores.(p= 0.00002). Prostate cancer with Gleason 7 score appears to be different than those with either 5-6 or 8-10 scores with regard to pretreatment PSA levels and tumor stages, and should be classified as a separate group.
