Benign prostat hiperplazili olgularda üretral kateterizasyonun serum prostat spesifik antijen değerleri üzerine etkisi: Pilot çalışma sonuçları

Özet
Uretral kateterin serum PSA duzeyine etkisinin arastmlmasi. Hastalar ve Yontem: Amaca yonelik olarak bir pilot galisma planland'i. Benign prostat hiperplazisi igin gerekli tetkiklerden gegerek operasyon planlanan 14 olgu kisa sureli ve kalici kateter takilmak Qzere randomize edildi. Uretral kateterizasyon oncesinde butun olgulardan kan ornegi alindi. Birinci gruba uretral kateter konduktan yarim saat sonra kateter cekildi (gegici kateter grubu), diger gruptaki olgularda ise katfter 21 gun sureyle yerinde tutuldu (kalici kateter grubu). Her iki gruptaki olgulardan kateterizasyon isleminden*2 saat sonra ve Vinci, 3'uncu, 5'inci, 7'nci, 10'uncu, 14'uncu, 17'nci ve 21'inci gunlerde kan ornekleri alindi. Alman kan omekleri ayni anda santrifuj edilerek -30° C'da dondurularak saklandi. Dondurulmu§ serumlar ayni anda cozduriilerek ayni kit ile PSA diizeyleri gali§ildi. Bulgular: Henki grup yas ve i§lem oncesi PSA duzeyleri agisindan farkli degildi. Her iki grupta da galisma suresince olgulen serum PSA duzeylerinde istatistiksel oneme ula§an higbir fark saptanmadi. Ayrica gruplar arasi kar§ila§tirmalarda da fark gozlenmedi. Sonug: Bu pilot galismamn bulgulan uretral kateterli bir olguda serum PSA yukseligi nedeninin primer olarak prostat bezi igerisinde aranmasi gerektigini dusundurmektedir.
Abstract
Objective: To investigate the effect of urethral catheterisation on serum prostate-specific antigen levels. Patients and Methods: A pilot study was planned to realize this aim. Fourteen patients awaiting surgery for benign prostatic hyperplasia were randomised into either short-term (for half an hour catheterisation) or long-term (for 21 days catheterisation) urethral catheterisation groups. Blood samples were drawn before urethral catheterisation, as well as 2 hours after and at the 1st, 3rd, 5th, 7th, 10th, 14th, 17th and 21st days for PSA assay. All. samples were centrifuged and the sera were stored at -30° C to be assayed altogether. The samples were thawed and processed for PSA by using a particular kit. Results: The two groups were comparable with respect to age and initial PSA values. Statistically significant difference was noted neither within nor between the two groups. Conclusion: The results of the present pilot study suggest that an elevated PSA value in a patient with urethral catheter should be an indication for prompt evaluation to exclude primary prostatic pathology.
