Radikal sistoprostatektomi sonrasında gelişen cinsel işlev bozuklukları

Özet
Amaç: Lokal invazif mesane kanseri nedeni ile radikal Sistoprostatektomi ve çeşitli tiplerde üriner diversiyon operasyonu uygulanan hastalarda postoperatif dönemde görülen cinsel işlev bozuklukları araştırmak. Yöntem: Ocak 1996 ve Aralık 2001 tarihleri arasında radikal Sistoprostatektomi ve üriner diversiyon operasyonu uygulanmış 26 hasta değerlendirildi. Hastalara cinsel işlevlerini sorgulayan anket, birebir görüşme ile soru-cevap tekniğine uygun olarak aynı hekim tarafından dolduruldu. Anket medikal ve demografik bilgileri, preoperatif ve postoperatif cinsel işlevleri sorgulayan 20 sorudan oluşuyordu. Bulgular: Postoperatif dönemde hastaların %52'sinde cinsel istek azalması ve %92'sinde erektil işlev bozukluğu saptandı. Hastaların 16'sı (% 64) postoperatif dönemde kuru orgazm yaşayabildiğini belirtti, ancak bu hastaların tamamında orgazm derecesinde azalma saptandı. Cinsel yaşantıdan tatmin 3 hastada (%11.5) minimal düzeyde olmak üzere tüm hastalarda azalmıştı. Sonuç: Radikal Sistoprostatektomi geçiren hastaların cinsel işlevlerinde önemli oranda bozulma meydana gelmektedir. Bu nedenle cerrahi tedavi planlanırken cinsel işlevler göz önüne alınmalıdır. Postoperatif dönemde aıtin olarak her hasta cinsel işlev yönünden değerlendirilmeli ve psikiyatrik destek dahil olmak üzere gerekli cinsel tedavi uygulanmalıdır.
Abstract
Objective: To investigate postoperative sexual dysfunction in locally invasive bladder cancer patients after radical cystopros-tatectomy and various types of urinary diversion. Methods: A total of 26 consecutive male patients who underwent radical Cystoprostatectomy and urinary diversion between January 1996 and December 2001 were evaluated. A sexual function questionnaire was completed by the same physician with interviews using question-answer technique. The questionnaire consisted of 20 questions including medical and demographic data, preoperative and postoperative sexual functions. Results: Postope-rative incidences for decreased sexual desire and erectile dysfunction were noted as 52% and 92%, respectively. Sixteen patients (64%) expressed that they could achieve dry orgasms, however all had decrease in the degree of orgasms. Although minimal in 3 cases (11.5%) overall satisfaction in sexual life was decreased in all patients. Conclusions: Sexual functions deteriorate significantly after radical Cystoprostatectomy. The impact of surgical treatment on sexual function should be considered during treatment plans. Postoperatively, all patients should be routinely evaluated for sexual function and considered for sexual therapy including psychological counseling.
