Metilen mavisi ile in vivo boyanmanın mesane tümörünün sistoskopik tanısındaki değeri

Mesane tümörünün tanısında kullanışlı bir yöntem olup olmadığını saptamak amacıyla mesane tümörlü ve/veya hematüri etyolojisi araştırılan 43 olguda sistoskopi ve biyopsi öncesinde % 0.1'lik metilen mavisi ile boyama yapıldı. Malign lezyonların, sistitler (p< 0.05) ve normal mesane mukozasından (p< 0.0001) ve sistitlerin de normal mesane mukozasından (p< 0.05) daha fazla metilen mavisi ile boyandığı saptandı. Boyalı sahalardan alınan biyopsilerde anormal mesane mukozası saptama oranı % 65.3 iken, boyasjz sahalardan alınanlarda % 33.3 idi. Yöntemin yalancı pozitiflik oranı % 49, yalancı negatiflik oranı % 22, sensitivitesi % 78, spesifisitesi ise % 51 olarak bulundu. Bu çalışmanın bulgularına göre metilen mavisi ile boyama yöntemi mesanedeki malign lezyonların saptanmasnda oldukça başarılı, basit ve ucuz bir tekniktir.
Abstract
Value of Cystoscopic Diagnosis of Bladder Tumors by In Vivo Staining With Methylene Blue- To determine the feasibility of in vivo staining with methylene blue in the diagnosis of bladder tumors, staining with 0.1 % methylene blue was performed before cystoscopy and biopsy on 43 patients who had bladder tumors or who were under investigation for the etiology ofhematuria. It was determined that malignant lesions were stained with methylene blue more than cystitis (p< 0.05) and normal bladder mucosa (p< 0.001) and also cystitis was stained more than normal bladder mucosa (p< 0.05). While the ratio of abnormal bladder mucosa detection in the biopsies obtained from stained areas was 65.3 %, it was 33.3 % when biopsies were taken from the unstained areas. With this method the false positive and false negative rates were found as 49 % and 22 %, respectively, while 78 % sensitivity and 51% specificity rates were obtained. According to the findings of the present study, methylene blue staining is a quite successful, easy and cost-effective procedure in the diagnosis of malignant lesions in the bladder.
