Prostat kanserli olguların ilk biyopsisinde gleason skoru 9 ve 10 saptanmasının evre ile ilişkisi

Özet
Prostat kanserinin diferansiasyonunu belirlemede en sık kullanılan Gleason skorlama sistemi prostat kanserinin biyolojik davranışını ve tedavi planını etkileyen en önemli parametrelerden biridir. Yeni bir yaklaşıma göre ilk biyopside Gleason skoru 9 ve 10 saptanan olguların diğer evrelendirme tetkikleri dikkate alınmaksızın ileri evre prostat kanseri olarak kabul edilmesi gerektiği savunulmaktadır. Mart 1995'ten bu yana biyopsi ile prostat kanseri tanısı konulan ve ilk biyopside Gleason skoru 9,10 bulunan 33 olguda evrelendirme yapılarak tümör davranışı belirlenmeye çalışılmıştır. Klinik evrelendirme sonrasında 3 olgu (% 9.1) evre B prostat kanseri kabul edilmiş, 30 olguda (% 90.9) ise tümörün prostat dışına yayıldığı belirlenmiştir. Radikal prostatektomi sonrasında önceden lokalize prostat kanseri düşünülen 3 olgunun da C evresinde olduğu görülmüştür. Son patolojik evrelendirme sonrasında tümörün 12 olguda (% 36.4) C evresinde, 21 olguda (% 63.6) D evresinde olduğu belirlenmiştir. Sonuç olarak, ilk biyopside Gleason skoru 9 ve 10 olan olguların hiçbirinde tümör prostata sınırlı bulunmamış ve bu tümörlerin erken evrede yakalanma olasılıklarının düşük olduğu kanısına varılmıştır.
Abstract
The Gleason grading system is widely used to predict the biological behavior of prostatic cancer and remains as the most important parameter to plan the treatment. A new concept proposes that patients with Gleason scores 9 and 10 on initial biopsy specimens should be considered as advanced prostate cancer regardless of the clinical stage. In this respect, 33 prostate cancer patients with initial Gleason scores of 9 and 10 were evaluated for the biological behavior of the tumor. After clinical staging 3 patients (9.1 %) were classified as stage B while 30 patients (90.9 %) were thought to have extraprostatic disease. After radical prostatectomy the 3 patients who were clinically considered as stage B were found to have stage C disease. The final pathological staging revealed that 12 patients (36.4 %) were in stage C in and 21 patients (63.6 %) had stage D disease. In conclusion, none of the patients with initial Gleason 9 and 10 biopsy scores was found to have organ confined prostate cancer. It appears that these tumors are hardly found in early stages.
