Özet

Bu çalışma, kanatlılarda mevcut olan doğal üreterointestinal formun morfolojik, fonksiyonel ve histolojik özelliklerinin belirlenmesi ve elde edilecek bilgilerin insanlarda uygulanan rektosigmoid poş cerrahisinde tekniğin gelişmesi ve komplikasyonların azaltılmasına katkı sağlayabileceği düşüncesiyle planlandı. Çalışmada ortalama ağırlığı 1870±88 g olan 30 adet tavuk (Gallus domesticus, 26 adet broiler, 4 adet yumurtacı) kulanıldı. Kloakanın histolojik ve morfolojik değerlendirilmesi yapıldı. Antero-posterior grafilerle üreteral reflü değerlendirildi. Koprodeum ile ürodeum,arasında plika koproürodealis, ürodeum ile proktodeum arasında plika üroproktodealis saptandı. Açılım bölgesine yakın konumdaki hem üreterin hem de ürodeumdaki bu kas demetlerinin yerleşimi güçlü bir sfinkter görünümündeydi. Radyopak madde verilerek elde edilen kloakal radyogramlara göre hiçbir olguda üreteral reflü saptanmadı. Kanatlılarda doğal olarak var olan ürointestinal yapı, doğal bir antireflü üreterointestinal açılıma sahiptir. Dışkı ve idrarın birbirine karışmasını ve artan basıncın üst üriner sisteme yansımasını engelleyen ayrıca üst barsak segmentlerine idrar kaçışını önleyen koproürodeal katlantı ürointestinal diversiyonlarda arzu edilen birçok özelliği doğal olarak sağlamaktadır.

Abstract

The present study was planned to investigate the morphological, functional and histological properties of an inborn ureterocloacal form in birds as well as to provide appropriate information that may contribute to the development of and decrasing complications in rectosigmoidal pouch surgery. Thirty chickens (Gallus domesticus, 26 broilers, 4 layers) with a mean weight of 1870±88 g were used in the study. Histological and morphological evaluation of the cloaca was performed. Ureteral reflux was evaluated by antero-posterior radiograms. Histomorphological examination revealed plica coprourodealis between the coprodeum and urodeum and the plica uroproctodealis between the urodeum and proctodeum. The arrangement of muscle fibers around the opening area both in the ureter and urodeum suggested a strong sphincteric structure. No ureteral reflux was detected according to cloacal radiograms with contrast material. The innate urointestinal structure in birds has a natural anti-reflux ureterointestinal openning. The coprourodeal fold that prevents mixing of faeces and urine, inhibits deleterious effects of increasing pressure on the upper urinary tract and hinders reflux of urine to the proximal intestinal segments, has naturally provided many of the desired properties in urointestinal diversions.
