Özet

Bu çalışmada 5 alfa redüktaz inhibitörü finasterid ile alfa adrenerjik reseptör blokörü doksazosinin benign prostat hiperplazisine bağlı infravezikal obstrüksiyonun tedavisindeki yeri araştırılmıştır. Çalışmaya orta derecede veya şiddetli prostatizm yakınmaları olan, maksimum idrar akım hızları <15 ml/s veya basınç akım çalışması ile obstrüksiyonu kanıtlanmış, rezidüel idrar miktarı <350 mi olan 65 hasta dahil edildi ve rastgele üç gruba ayrıldı. Birinci grupta (n= 36) 6 ay süreyle 5 mg/gün finasterid tedavisi uygulandı. İkinci grupta (n= 19) 6 ay süreyle 4 mg/gün doksazosin tedavisi uygulandı. Üçüncü gruba (n= 10) 3 ay süreyle plasebo verildi. Tüm hastalarda tedavi öncesi ve sonrasında semptom skorlaması (AUA), dijital rektal muayene ve pelvik ultrasonografi ile prostat büyüklüğü, maksimum idrar akım hızı ve rezidüel idrar miktarı tayini yapıldı. Altı aylık tedavi sonunda finasterid grubunda semptom skoru 7.03 puvan azaldı (p= 0.0001), maksimum akım hızı 2.96 ml/s arttı (p= 0.020), prostat büyüklüğünde % 16.9 oranında küçülme oldu (p= 0.006) ve rezidüel idrar miktarında ise 16.5 mi azalma saptandı (p= 0.243). Doksazosin grubunda ise semptom skorunda 7 puvan azalma (p= 0.0001), maksimum akım hızında 1.97 ml/s artış (p= 0.025) ve rezidüel idrar miktarında 17.1 mi düşüş (p= 0.491) elde edilirken prostat büyüklüğünde % 0.18 artış oldu. Cerrahi tedavi kadar etkili olmasa bile gerek finasterid gerekse doksazosin tedavisinin gözleyerek beklemeye göre çok daha etkili birer tedavi seçeneği olduğu düşünülmüştür.

Abstract

Finasteride and Doxazosin in Medical Treatment of Benign Prostatic Hyperplasia- This study was planned to investigate the place of 5 alpha reductase inhibitor finasteride and alpha adrenergic receptor blocker doxazosin in the treatment of infravesical obstruction due to benign prostatic hyperplasia. Sixty-five patients with moderate to severe prostatism symptoms, who had either maximum urinary floru rates of <15 ml/s or proven obstruction by pressure-floıv studies and residual urine volumes of < 350 mi ıvere included into the study and randomly assigned into three groups.The first group of patients (n- 36) ıvere given 5 mg/day finasteride for six months. The second group (n= 19) ıvas treated ıvith 4 mg/day doxazosinfor six months and the third group (n= 10) received placebofor three months. Ali patients ıvere assessed by determination ofsymptom scores (AUA), prostatic enlargement by digital rectal examination and pelvic ultrasonography, maximum urinary floıv rates and residual urine volumes before and after treatment. After six months of therapy, 7.03 points decrease in symptom score (p= 0.0001), 2.96 ml/s increase in maximum urinary floıv rate (p= 0.020), 16.9 % decrease in prostatic volume (p= 0.006) and 16.5 mi decrease in residual urine (p= 0.243) ıvere attained in the finasteride group. in the doxazosin group, the corresponding values ıvere 7 points decrease in symptom score (p= 0.0001), 1.97 ml/s increase in maximum urinary floıv rate (p= 0.025), 17.1 mi decrease in residual urine volume (p= 0.491) and 0.18 % increase in prostatic volume. Although their efficacy remains far from that of surgical treatment, both finasteride and doxazosin appear as effective alternatives for treatment ıvhen compared to ıvatchful ıvaiting.
