İntrakavernöz enjeksiyon uygulanan impotans olgularında üçlü kombinasyonun etkinliği

Özet

İmpotans tedavisinde intrakavernöz papaverin veya papaverin artı fentolamin enjeksiyonlarına yanıt alınamayan olgularda papaverin artı fentolamin artı prostaglandin Ei kombinasyonunun etkinliği incelen¬di. Renkli Doppler ulltrasonografi ile hafif ve orta dereceli arteriyel yetmezlik saptanan 45 (grup I), renkli Doppler ulltrasonografi ve dinamik infüzyon kavemozometri ve kavemozografisi ile hafif ve orta dereceli venooklusif disfonksiyon saptanan 40 (grup II), venooklusif disfonksiyon nedeniyle derin dorsal ven ligasyonu uygulanan 15 (grup III), ileri derecede arteriyel yetmezliği olan 25 (grup IV), ileri derecede venooklusif disfonksiyonu olan 18 (grup V) ve miks tip vasküler yetmezliği olan 29 (grup VI) olgu çalışma gruplarını oluşturdu. Daha önce 60 mg papaverin ve 40 mg papaverin artı 2.5 mg fentolaminle koitusa yeterli ereksiyon sağlanamayan bu olgulara 7.5 mg papaverin artı 0.4 mg fentolamin artı 4 \ıg prostaglandin Ei kombinasyonu uygulandı. Grup l'deki 45 olgudan 34'ü (% 75), grup ll'deki 40 olgudan 29'u (% 72), grup lll'teki 15 olgudan 8'i (% 53), grup IV'teki 25 olgudan 9'u (% 36), grup V'teki 18 olgudan 6'sı (% 33) ve grup Vl'daki gruptaki 29 olgudan 6'sında (% 20) üçlü kombinasyon enjeksiyonu ile yeterli ereksiyon sağlandı. İntrakavernöz papaverin veya papaverin artı fentolamin enjeksiyonla-rıyla koitusa yeterli ereksiyon sağlanamayan,.özellikle hafif ve orta derece arteriyel yetmezlik veya venooklusif disfonksiyon olgularında papaverin artı fentolamin artı prostaglandin Ei kombinasyonunun etkili olabileceği kanısına varılmıştır.

Abstract

Efficacy of Tri-Mix Combination of Intracavernous Injections in Impotence-The efficacy of papaverine plus phentolamine plus prostaglandin Ej combination in impotent patients unresponsive to intracavernous papaverine or papaverine plus phentolamine injections zuas investigated. The study included 45 patients xoith mild to moderate arterial insufficiency diagnosed with color Doppler ultrasonography (group I), 40 patients with mild to moderate venooclusive dysfunction diagnosed xoith color Doppler ultrasonography and dynamic infusion cavernosometry and cavernosography (group II), 15 patients luho undenuent deep dorsal vein ligation because of venooclusive dysfunction (group III), 25 patients with severe arterial insufficiency (group IV), 18 patients with severe venooclusive dysfunction (group V), and 29 patients with mixed type vascular insufficiency (group VI). These patients who remained unresponsive to either 60 mg papaverine or 40 mg papaverine plus 2.5 mg phentolamine zoere tested again by using intarcavernous injections of 7.5 mg papaverine plus 0.4 mg phentolamine plus 4 /.ig prostaglandin E\. Thirty four cases out of 45 (75 %) in Group I, 29 of 40 (72 %) in Group II, 8 of 15 (53 %) in Group III, 9 of 25 (36 %) in Group IV, 6 of 18 (33 %) in Group V and 6 of 29 (20 %) in Group VI responded with sufficient erections for intercourse to this tri-mix combination. The tri-mix combination of papaverine plus phentolamine plus prostaglandin Ej appears to be effective in terms of attaining sufficent erections for coitus particularly in cases with mild to moderate arterial insufficiency or venooclusive dysfunction who are unresponsive to intracavernous papaverine or papaverine, plus phentolamine injections.
