Benign prostat hiperplazisinin cerrahi tedavisinde transüretral elektrovaporizasyon: Uzun dönem izlem sonuçları

Son yıllarda benign prostat hiperplazisinin cerrahi tedavisinde standart transüretral rezeksiyona (TURP) alternatif bir yöntem olarak ortaya çıkan transüretral elektrovaporizasyonun (TVP) etkinliğini ve uzun dönem sonuçlarını değerlendirmek için semptomatik benign prostat hiperplazisi olan toplam 66 hasta TURP veya TVP yapılmak üzere iki gruba randomize edildi. Hastalar operasyon öncesinde maksimum idrar akım hızı (Qmax), semptom skoru ve transrektal ultrasonografi ile değerlendirildi. İrrigasyon sıvısındaki hemoglobin miktarı ve rektum duvarına yerleştirilen bir elektrot ile tüm hastaların transrektal sıcaklık ölçümleri alındı. TVP grubunda transrektal sıcaklık artışı, ortalama 0.83° C (0:53 ile 1.27° C arasında) daha fazla olarak saptanmasına karşın, buna ait bir komplikasyon gözlenmedi. Ölçülen kan kaybı ise TURP grubunda belirgin olarak daha fazla idi (p< 0.001). Hastaların 3, 6 ve 12'nci aylardaki kontrollerinde semptom skoru ve Qmax düzelmeleri her iki grupta da tatminkârdı. Düşük morbiditesi ve başarılı uzun dönem sonuçları ile TVP, transüretral rezeksiyona iyi bir alternatif olarak görünmektedir.
Abstract
To evaluate the efficacy and long term results of transurethral electrovaporization of the prostate (TVP) which has recently been introduced as an alternative to standard transurethral resection (TURP) in the treatment of benign prostatic hyperplasia, 66 patients with symptomatic benign prostatic hyperplasia were randomly divided into tiuo groups for TURF or TVP. The patients were preoperatively assessed with measurement of maximum urinary flow rates (Qmax), determination of symptom scores and transrectal ultrasonography. Hemoglobin concentrations in the irrigation fluid and transrectal temperature measurements by means of a rectal wall electrode were obtained in all patients. Although the transrectal temperatures in the TVP group were higher with a mean value of 0.83 ° C (range; 0.53-1.27°'Q, no associated complications were determined. On the other hand, blood loss was significantly higher in the TURP group (p< 0.001). Postoperative followup at 3, 6 and 12 months after surgery revealed satisfactory results in both groups. With its low morbidity and successful long term results TVP appears as a good alternative to TURP.
