Özet

Üretra darlığı, tekrarlayan operasyonlara gereksinim duyulmasından dolayı klinisyenler için önemli bir sorundur. Bu çalışmada son 15 yıl içinde üretra darlığı nedeniyle tedavi edilen 373 hasta retrospektif olarak değerlendirilmiştir. Postprostatektomi ve üretral manipülasyonlar üretra darlığı etyolojisinde en önemli faktörlerdi. Olguların % 75' inde darlık lokalizasyonu posterior üretrada bulundu. En az 1 yıllık izlemi olan hastalarda ortalama nüks oranı % 42.6 olarak belirendi. Bu serideki olgular, üretra darlığının tedavisinin seçiminde, darlığın lokalizasyonu, etyolojisi, uzunluğu ve birlikte başka bir patoloji olup olmamasının önemli olduğunu düşündürmek-tedir. Seçilmiş olgularda üretra darlıklarının nüksünün azaltılması için tedavi sonrasında temiz aralıklı kateterizasyonla yumuşak dilatasyonlar veya üretral stent gibi yardımcı yöntemlerin yararlı olabileceği anlaşılmaktadır.

Abstract

Urethral strictures remain as a difficult problem for the clinician as they usually require multiple operations with unrewarding results. We retrospectively reviewed 373 patients with urethral strictures treated with different modalities during the last 15 years. Prostatectomy and urethral manipulations were the most important etiologic factors. The strictures were localised in posterior urethra in 75 % of the cases. The mean recurrence rate was 42.6 % in patients who had at least one year follow-up. The findings of the present series suggest that several factors, including stricture localisation, etiology, stricture length and presence of coincident pathologies, have considerable impact on the choice of treatment for urethral strictures. In selective cases, post-treatment auxiliary procedures such as clean intermittent self catheterisation or urethral stents seem to be necessary for decreasing the rate of recurrences.
