Özet

Stres İnkontinans detrüsör kontraksiyonu veya mesanenin aşırı distansiyonu olmaksızın, karın içindeki basınç artışına bağlı istemsiz idrar kaçırmadır. Tedavisinde farklı birçok yönteme karşın hangi tür cerrahi girişimin en uygun olduğu konusunda görüş birliği yoktur. Bu klinik çalışmada Tip III stres inkontinansı olan 12 hastaya uygulanan pubovajinal sling işleminin sonuçları sunulmuştur. Yöntemin uygulandığı 12 hastadan 11'i takip edilebildi. İzlem süresi 4 ay ile 2 yıl arasında değişmekteydi (ortalama, 11 ay). İzlenen hastaların tümünde şikayetler kayboldu. Hastaların ortalama yaşı 53.5 yıl (40 ile 71 yıl arasında), ortalama doğum sayısı ise 3.3 idi. Oniki hastanın 4'ü daha önce aynı sebeple, ameliyat geçirmişti. Hastanede kalış süresi ortalama 5.5 gündü. Abdominal kaçırma noktası ortalama basınç değeri 54.6 cmHa2O idi. Bir hastada mesane perforasyonu oluştu. Operasyon sonrasında iki hastada detrüsör instabilitesi diğer iki hastada postoperatif erken dönemde retansiyon gelişti. Bu çalışmanın bulguları, olgu sayısının azlığına karşın, pubovajinal sling operasyonun özellikle komplike stres inkontinans tedavisinde uygulanması gereken yöntem olduğu fikrini desteklemektedir.

Abstract

Stress urinary incontinence is defined as urinary leakage secondary to an increase in abdominal pressure in the absence of either a detrusor contraction or an overdistended bladder. Although many different surgical techniques have been described, there is no concensus about a single surgical procedure of choice. In this clinical study, we present the results of pubovaginal sling operations performed in 12 patients with stress urinary incontinence. Eleven of 12 patients remained infollowup. Meanfollowup time was 11 months (range; 4-24 months). All of the patients infollowup were symptom free. The mean patient age was 53.5 years (range; 40-71 years) and the mean parity was 3.3. Four patients had previous operations for the same reason. Average duration of hospitalization was 5.5. days. Mean abdominal leak point pressure level was 54.6 cmH20. Bladder perforation occurred in one patient during the procedure. Postoperative detrusor instability appeared in 2 patients and urinary retention was seen in another two. Despite the limited number of cases findings of the present study support the idea that pubovaginal sling operation is the method of choice in the treatment of complicated cases of stress urinary incontinence.
