Özet

Bu çalışmanın amacı kobaylarda intravezikal BCG (bacillus Calmette-Guerin) verilmesiyle oluşturulan mikobakteriyeminin polimeraz zincir reaksiyonu (PZR) yöntemi ile gösterilmesidir. Ayrıca, mikobakteriyeminin tepe zamanı ve uygulanan BCG dozunun travmatize edilen ve edilmeyen mesanelerdeki etkisi araştırılmış, rutin kültür yöntemleri ve PZR'nin duyarlılığı karşılaştırmıştır. Toplam 50 kobay 5'erli gruplara ayrılarak mesanesi travmatize edilen ve edilmeyen deneklere beş farklı doz Connaught BCG (0, 0.069, 0.69, 6.9 ve 69 mg) 1 ml hacimle intravezikal olarak uygulandı ve bunu izleyerek 0, 5,15, 30 ve 60'ıncı dakikalarda PZR ve kültür için 0.5 ml kan örneği alındı. Travmatize grupta intravezikal 69 mg BCG verilen hayvanlardan 30'uncu dakikada alınan kanların 3'ünde (% 60) ve 60'ıncı dakikada alınan kanların 4'ünde (% 80) PZR sonucu pozitif bulunmuştur. Diğer örneklerin hepsinde PZR sonucu negatifti ve kültürlerde üreme olmadı. Bununla birlikte PZR sonucu pozitif olan 7 hastanın da kültürlerinde üreme olmadı. Mikobakteriyemi yalnızca travmatize edilmiş mesane varlığında ve yüksek doz BCG verilmesiyle oluşmaktadır. Mikobakteriyemiyi saptamada PZR son derece duyarlı ve hızlı bir tanı yöntemidir.

Abstract

The aim of this study was to detect mycobacteremia produced with intravesical instillation of BCG (bacillus Calmette-Guerin) in guinea pigs, by means of polymerase chain reaction (PCR) method, We also investigated the peak time of mycobacteremia and the role of BCG dose in traumatised and nontraumatised bladders. The sensitivities of routine cultures and PCR were also compared. Five different doses of Connaught BCG (0, 0.069, 0.69, 6.9 and 69 mg) all in 1 ml volume, were instilled into 5 traumatised and 5 nontraumatised bladders for each dosage, thus using a total of 50 female guinea pigs. Following instillations blood samples were drawn at 0,5,15, 30 and 60th minutes for PCR and routine culture. In the traumatised group, 3 of 5 samples (60 %) obtained 30 minutes after instillation of 69 mg BCG were PCR positive. The PCR positive samples were 4 in 5 (80 %) when samples were obtained at the 60th minute. All of the remaining samples were negative for PCR and routine culture. The routine cultures remained also negative in all PCR positive samples. Mycobacteremia occures only when the bladder is traumatised and a high dose of BCG is used. PCR is a highly sensitive and rapid diagnostic method for the detection of mycobacteremia.
