Özet
Bu prospektif kör çalışmada mesane tümörü tanı ve izleminde, sistoskopi ve biopsiye göre Bard BTA (bladder tumor antigen) testinin güvenilirliği araştırıldı. Yirmisi makroskobik hematüri ile başvuran ve mesane tümörü öyküsü olmayan, 37'si yüzeyel mesane tümörü nedeniyle sistoskopi ile izlenen 57 olgu çalışmaya alındı. Tüm olgulara önce BTA testi uygulanıp ardından ültrasonografi, intravenöz piyelografi ve bilgisayarlı tomografi-den oluşan görüntüleme yöntemleri ve genel anestezi altında sistoskopi ile rastgele biyopsi veya görünür tümör saptananlarda transüretral rezeksiyon yapıldı. Bulgular, BTA testi sonuçları ile karşılaştırıldı. Mesane tümörü tanısında BTA testinin duyarlılık, özgüllük, pozitif kestirim değeri ve doğru tanı koyma oranları, sırasıyla % 78, % 71, % 57 ve % 73 idi. Mesane tümörü tanısı ve yüzeyel mesane tümörlerinin izlenmesinde BARD- BTA testi sistoskopi ve biyopsinin yerini alabilecek güvenilirlikten uzak gibi görünmektedir.
Abstract
— Reliability of the Bard BTA Test in the Diagnosis of Bladder Tumors-In this prospective blind study, the reliability of Bard BTA (bladder tumor antigen) test was investigated with regard to cystoscopy and biopsy findings in the diagnosis andfollowup of bladder cancer. Of 57 patients included into this study, 20 cases admitted with the complaint of macroscopic hematuria without known history of bladder cancer and 37 cases were onfollowup cystoscopy protocol for known superficial bladder tumour. All subjects initially underwent BTA test followed by diagnostic imaging techniques including ultrasonography, intravenous urography and computerized tomography and cystoscopy under general anesthesia with either random biopsies or transurethral resection when a tumor was detected. The results of conventional workup were compared to those obtained by the BTA test. The sensitivity, specificity, positive predictive value and accuracy rates of BTA test were 78 %, 71 %, 57 % and 73 %, respectively. The reliability of the BTA test at the moment appears to be inadequate to substitute cystoscopy and biopsy in the diagnosis andfollowup of bladder tumors.
