Özet
Amag: intrinsik sfinkter yetmezligine bagh stres inkontinansi olan kadinlarda periuretral teflon enjeksiyonu etkinliginin incelenmesi. Hastalarve Yontem: Ocak 1997 ile Mart 1998 tarihleri arasinda toplam 20 hastaya periuretral teflon enjeksiyonu uygulandi. intrinsik sfinkterik yetmezligin tanisi igin uretral basing profili, abdominal kagirma noktasi basinci ve gereken olgularda videourodinamik galisma kullanildi. Tedaviye yanit kullanflan pet sayisi .ve inkontinans derecesindeki azalmaya gore degerlendirildi. Yanit alinan ancak tam duzelmeyen hastalara ikinci defa enjeksiyon yapildi. Bulgular:Tedaviden once ortalama 6 (3 ile10 arasinda) olan pet sayisi tedaviden sonra 3'e indi. Toplam 8 hasta (%JtO) tam tedavi oldu, 6 hastada (% 30) kullanilan pet sayisi azaldi. Bu 6 hastanm 3'unde ikinci enjeksiyon ile |ullanilan pet sayisi daha da azaldi. Alti hastada (% 30) herhangi bir duzelme olmadi. Kullanilan ortalama teflon miktari 12 ml olarak belirlendi. Sonug: Periuretral teflon enjeksiyonu kadinlarda sfinkter yetmezligine bagli stres inkontinans igin kullanilabilecek etkili bir tedavi yontemidir.
Abstract
To investigate the efficacy of periurethral teflon injections in women with stress urinary incontinence due to intrinsic sphincteric deficiency. Patients and Methods: Between January 1997 and March 1998 periurethral teflon injections were done in 20 patients. The diagnosis for intrinsic sphincteric deficiency was made by using urethral pressure profile, abdominal leak point pressure and videourodynamic studies when needed. Response to treatment was evaluated by the amount of decrease in daily pad use and decrease in the clinical grades. Secondary injections were performed in patients who ameliorated without total cure. Results: The mean number of daily used pads decreased from the pretreatment level of 6 (range 3 to 10) to 3 after treatment. Complete cure was achieved in 8 patients (% 40) while 6 patients (% 30) showed improvement with decreased daily pad usage. Three out of these 6 patients showed further improvement as noted by decreased number of pads after second injections. The treatment failed in 6 patients (30 %). The mean amount of injected teflon was 12 ml. Conclusion: 'Periurethral teflon injections appear efficacious in the treatment of female stress incontinence due to intrinsic sphincteric deficiency.
