Özet

Amaç: Hemodiyaliz hastalarında damar girişi için oluşturulan arteriyovenöz fistüllerin açıklık oranlan ve bunu etkileyen faktörlerin değerlendirilmesi. Yöntem: Mayıs 1999 ile Haziran 2002 tarihleri arasında son dönem böbrek yetmezliği gelişmiş ve arteriyovenöz fistül açılmış olan 92 hasta retrospektif olarak değerlendirildi. Enfiye çukurundan 78, stiloid proçesin proksimalinden 22 ve antekübital bölgeden 16 olmak üzere toplam 116 girişim uygulandı. Bulgular: Enfiye çukurunda oluşturulan fistüllerde ilk 24 saatte %92 açıklık varken, 36 aylık takipte bu oran %42'ye geriledi. Aynı oranlar, stiloid çıkıntının proksimali için, sırasıyla, %100 ve %70, antekübital bölgede için, sırasıyla %100 ve %66 olarak belirlendi. Diyabetik hasta grubunda açıklık oranları düşerken, yaş ve cinse göre anlamlı farklar saptanmadı. Sonuç: Kronik böbrek yetmezliği olan ve hemodiyaliz uygulanan hastaların bu ihtiyacı yaşam boyu sürebilir. Tüm hastalarda ve özellikle de diyabetli olgularda vasküler giriş bölgeleri iyi korunmalıdır. Hemodiyaliz çalışanlarının konu hakkında bilgilendirilmesi ve uygun cerrahi teknik fistülün daha uzun süre kullanılmasına yardımcı olacaktır.

Abstract

Objective: To assess the outcome of arteriovenous fistula formations for vascular access in hemodialysis patients and to determine factors affecting the patency rates. Methods: Ninety-two patients with end stage renal disease who underwent arteriovenous fistula formation between May 1999 and June 2002 were retrospectively reviewed. The procedures were done in the snuff-box region in 78 patients, proximal to the stiloid process in 22 and in the antecubital region in 16 cases yielding a total of 116 operations. Results: While the patency rate was 92% at 24 hours for fistulas created in the snuff-box region this figure receded to 42% at 36 months. The corresponding figures for fistulas proximal to the stiloid process were 100% and 70%, respectively; and 100% and 66%, respectively for those in the antecubital region. While the patency rates were significantly worse in diabetics compared to non-diabetics, there were no significant differences with regard to sex and age of the patients. Conclusion: A vascular access may be lifelong necessary for a patient with chronic renal failure. For all hemodialysis patients and particularly in diabetics, potential vascular access sites should be protected carefully. A well information and education of hemodialysis personnel and application of proper surgical techniques are necessary for the availability and long-term survival of fistulas in these patients
