Serum testosteron ve serbest testosteron düzeyleri prostat kanserinin erken tanısında prostat spasifik antijen parametrelerinin etkinliğini artırıcı bir araç olabilir mi?

Özet

Amaç: Testosteronun, hem prostat kanseri tanısında, hem de prostat kanserinin patolojik evresini öngörmede etkin bir belirleyici olup olmadığının incelenmesi. Yöntem: Nisan ve Haziran 2002 tarihleri arasında prostat biyopsisi yapılan 44 hastanın, serum testosteron, serbest testosteron düzeyleri ile prostat spesifik antijen (PSA) parametreleri incelendi. Hastalardan transrektal ultrasonografi eşliğinde, periprostatik lokal anestezi ile 10 kadran biyopsi alındı. Bulgular: Kanser olan hastaların ortalama testosteron, serbest testosteron düzeyleri benign prostat hiperplazisi (BPH) olanlardan farklılık göstermedi. Kanser bulunan hastaların serbest PSA oranı (s/t PSA) ve PSA dansitesi (PSAD) değerlerinin patolojisi benign olanlardan anlamlı olarak farklı idi (sırasıyla, p= 0.04 ve p= 0.014). Serum PSA düzeyleri 4 ile 10 ng/mL arasında kanser varlığını belirlemede testosteron ve serbest testosteron düzeylerinin etkin olmadığı, bu hastalarda s/t PSA (p= 0.025) ve PSAD (p= 0.016) ölçümleri daha değerli bulundu. Ayrıca serbest ve total testosteron düzeyleri Gleason skoru yüksek ve düşük olan kanserler arasında farklı değildi. Bu-na karşın serbest testosteron oranı düşük olan hastaların biyopsilerinde kanser hacminin daha fazla olduğu saptandı. Sonuç: Serum testosteron ve serbest testosteron düzeylerinin, prostat kanserini öngörmede, PSA parametrelerine bir üstünlük sağlamadığı olmadığı anlaşılmaktadır. Diğer taraftan, serum serbest testosteron düzeyi düşük ve normal olan hastaların biyopsilerinde kanser hacmi farklılık göstermektedir.

Abstract

Objective: To investigate the role of testosterone both in the diagnosis and prediction of pathological stage of prostate cancer. Methods: The total and free testosterone levels and prostate specific antigen (PSA) parameters of 44 patients who underwent prostate biopsies between April and June 2002 were evaluated. Ten biopsies were taken under periprostatic local anesthesia with the guidance of transrectal ultrasonography. Results: The total and free testosterone levels of patients with prostate cancer and benign prostate hyperplasia were not significantly different. However the free PSA ratio (free/total PSA) and PSA den-sity (PSAD) of patients with prostate cancer and benign prostate hyperplasia showed statistical difference (p= 0.04 and p= 0.014, respectively). Serum testosterone levels were not helpful in predicting prostate cancer in patients with PSA levels between 4 and 10 ng/mL. However, the percent free PSA (p= 0.025) and PSAD (p= 0.016) values appeared to be more useful predictors of cancer in these patients. In addition, the free and total testosterone levels were comparable among cancers with higher and lower Gleason scores. However, cancer volumes in the biopsies of patients with lower free testosterone levels were higher. Conclusion: Determination of serum testosterone and free testosterone levels do not provide additional information over PSA parameters in the diagnosis of prostate cancer. Cancer volumes on biopsy specimens of patients with lower and hig-her free testosterone levels show differences.
