Özet

Malign melanom, deride pigment üreten hücreler olan melanositlerin malignitesi olup, göz, gastrointestinal sistem, leptomeninksler ile oral ve genital müközmembranlarda da olabilir. Tüm cilt kanserlerinin sadece %4’ünü oluştursa da dünyada cilt kanserine bağlı ölümlerin %74’ünün sebebi malign melanom metastazlarıdır. Melanom, daha çok lenfatik yolla olmakla birlikte lenfatik ve hematojen yolla metastaz yapar. Metastazları da sıklıkla deri, deri altı yağ dokusu ve lenf nodları gibi non visseral bölgelere olur. Ciltte primer melanomun metastatik melanomdan ayrılması prognoz açısından oldukça önemlidir. Histolojik kriterler bu ayırımın yapılmasını sağlamakla birlikte, en önemli bulgular klinik bilgi ile elde edilebilir. Bu olgu daha önce literatürde bildirilmeyen bir şekilde aynı lokalizasyonda, aynı anda, birden fazla malign melanom tipini bulundurduğu için bildirilmesi uygun görülmüş ve lezyonların primer mi oluştuğu yoksa metastatik mi olduğu literatür eşliğinde tartışılmıştır.

Abstract

Melanoma is a malignancy of pigment producing cells (melanocytes) located predominantly in the skin, but also found in eyes, gastrointestinal tract, leptomeninges, and oral and genital mucous membranes. While melanoma accounts for roughly 4% of all skin cancers, it is responsible for more than 74% of skin canser-related deaths worldwide. Melanoma is notable for its propensity to metastasize to any organ, either by lymphatics (the most common means) or through hematogenous spread. Metastases most commonly involve nonvisceral sites, such as the skin, subcutaneous fat, and lymph nodes, rather than visceral organs. The distinction of primary melanoma from metastatic melanoma in the skin is of considerable prognostic importance. Although several histologic criteria can facilitate this discrimination, clinical information is perhaps the most important determinant.
