Özet
Spinal disrafizm, embriyonal dorsal orta hat bölgesindeki yapıların birleşimindeki yetersizlik olarak ifade edilir. Gergin kord gizli spinal disrafizmin göstergelerinden biridir. Spinal disrafizmin kutanöz değişiklikleri lipom, dermal sinüs/kist, hipertrikoz, ‘dimple’, hiperpigmentas-yon, kapiller hemanjiom ve aplazia kutistir. Bu bulgular gizli nörolojik anomalilerin erken belirtisi olabilir. Spinal disrafizmden şüphelenilen hastalarda manyetik rezonans inceleme altın standart olarak düşünülmektedir. Gergin kordun erken tanısı önemlidir çünkü cerrahi ile geri dönüşümsüz nörolojik komplikasyonlar önlenebilir. Biz burada gergin kord ile ilişkili olarak aynı bölgede ‘faun tail’ (hayvan kuyruğu) ve kapiller malformasyonu olan genç bir kız hastayı sunuyoruz. Dermatologlar ve pediatristler benzer hastaları gören ilk klinisyen olabilirler ve spinal defektlerin altında yatan durumların farkında olmaları gerekir.
Abstract
Spinal dysraphism refers to incomplete fusion of midline structures of the embriyonal dorsal median region. Tethered cord is one of the markers of occult spinal dysraphism. Cutaneous changes of spinal dysraphism may include a lipoma, dermal sinus/cyst, hypertrichosis, dimple, hyperpigmentation, capillary hemangioma and aplasia cutis. These signs led to early recognution of the occult neurological abnormalities. Magnetic resonance imaging is considered to be the gold standard for investigating patients with suspected spinal dysraphism. Early diagnosis of a tethered cord is important, because surgery can prevent irreversible neurologic symptoms. We reported a young girl presented with faun tail and capillary malformation on same region associated with tethered cord. The dermatologists or pediatricians may be the first physicians to see such patients and should be aware of the potential underlying spinal defects.
