Dejeneratif lomber spondilolistezis ve lomber spondilozlu hastalarda fonksiyonel durum ve yaşam kalitesinin değerlendirilmesi

Özet
Amaç: Bu çalışmanın amacı dejeneratif lomber spondilo listezis (DLS) ve lomber spondiloz (LS) tanılı hastaların fonksiyonel durumunu ve yaşam kalitesini değerlendir mekti. Materyal ve Metot: Çalışmaya kronik bel ağrısı (>3 ay) şikayeti olan 45-65 yaş arası, 70 hasta dahil edildi. Hastalar DLS tanılı 39 hasta ve LS tanılı 31 hasta olarak iki gruba ayrıldı. Hastaların yaşları (yıl), vücut kitle indeksleri (VKİ, kg/m2 ), semptomları ve semptom süreleri (ay) kaydedildi. Lokomotor sistem ve nörolojik muaye neleri yapıldı. Radyolojik değerlendirme iki yönlü ve oblik lomber grafiler, gereken hastalarda manyetik rezonans görüntüleme ile yapıldı. Hastaların ağrısı Vizüel Analog Skala (VAS), fonksiyonel durumu “Modifiye Oswestry Indeksi” (MOI) ve yaşam kalitesi “Kısa Form-36” (Short Form-36/SF-36) Türkçe versiyonu ile değerlendirildi. Bulgular: DLS’li hastaların yaş ortalaması 52,51±6,76 yıl, LS’li hastaların yaş ortalaması 56.90±4.96 yıl idi. DLS’lilerde bel-bacak ağrısı, ekstansiyonda ağrı ve limi tasyon, hamstring gerginliğinin anlamlı düzeyde fazla olduğu not edildi. Fonksiyonel durum ve yaşam kalitesi karşılaştırıldığında her iki grup arasında farklılık yoktu. Sonuç: DLS’li hastalar LS’li hastalara göre daha erken yaşlarda başvurabilen, semptom ve bulguları daha fazla olan hastalar olsa da fonksiyonel durum ve yaşam kalitesini LS’li hastalara benzer şekilde etkilemektedirler. Bu durum LS’li hastaların analiz ve tedavisinin de aynı dikkat ve özen içinde yapılmasının gerekli olduğunu göstermiştir.
Abstract
Objective: The aim of this study was to evaluate functional status and quality of life in patients with degenerative lumbar spondylolisthesis and lumbar spondylosis. Materials and Methods: A total of 70 patients between the ages of 45-65 years with chronic low back pain (>3 months) were included in this study. Patients were divided into two groups; 39 patients with degenerative lumbar spondylolisthesis and 31 patients with lumbar spondylosis. All patients’ age (year), body mass index (BMI, kg/m2 ), symptoms and duration of symptoms (month) were noted. Locomotor and neurological examination were assessed. Radiological evaluation was performed with two sides and oblique lumbar radiographs and magnetic resonance imaging if needed. Pain intensity, functional status and quality of life were evaluated by Visual Analog Scale (VAS), Modified Oswestry Index (MOI) and Short Form-36 Turkish version (Short Form-36/SF-36). Results: The mean ages of patients with DLS and LS were 52.51±6.76 years and 56.90±4.96 years, respectively. Low back pain, radiation of the pain to the lower limbs, worsening of the pain on extension, limitation during extension and hamstring tightness were significantly higher in patients with DLS. There was no difference between the groups in terms of functional status and quality of life. Conclusion: Although, patients with DLS may apply at younger ages with more prominent symptoms and findings, their functional satus and quality of life are affected similar to that of LS patients. This finding shows that the patients with LS should be investigated and treated with the same attention and care as DLS patients.
