Özet

Amaç: Bu çal ı şman ı n amac ı semikantitatif de ğ erlendirmenin, manyetik rezonans görüntülemesinde meme lezyonlar ı n ı n malign-benign ayı r ı m ı nda özgüllü ğ ü artı rma ve objektif veriler sa ğ layabilme konusundaki etkinliğ ini de ğ erlendirmektir. Materyal ve Metot: De ğ erlendirmeye 95 lezyon al ı nd ı . Her bir lezyon için TIC (“Time intensity curve”- zaman- kontrast e ğ risi) e ğ rileri olu şturuldu. Postprosesing işlemlerinden yararlan ı larak TIC e ğ rilerinden tüm lezyonlar ı n semikantitatif analiz de ğ erleri elde edildi. Ayrı ca kinetik e ğ rilerin kalitatif de ğ erlendirilmesinde TIC e ğ rileri geç evredeki seyrine göre, progresif, plato ve yı kanma (veya “wash-out”) olarak üçe ayr ı ld ı . Elde edilen veriler istatistiksel de ğ erlendirmeye katı ld ı . Bulgular: ‘‘Time-to-peak’’ de ğ erleri malign lezyonlar için daha k ı sa sürmekte ve istatistiksel olarak anlaml ı sonuç vermektedir (p=0,01). Diğ er kantitatif parametrelerde malign ve benign lezyon ayı r ı m ı nda kullan ı labilecek anlaml ı istatistiksel veri sa ğ lanmam ı ştı r. Elde ettiğ imiz e ğ ri tiplerinin lezyonlar ı n ayı rı m ı nda tan ı sal de ğ eri oldu ğ unu görmekteyiz. Sonuçlar: Çal ı ş mam ı z sonucunda, TIC e ğ ri paterninin de ğ erlendirilmesi ve semikantitatif parametrelerden Tpeak de ğ erinin lezyonları n benign-malign ayı rı m ı nda istatistiksel olarak anlaml ı oldu ğ unu bulduk.

Abstract

Background: The aim of this study is to assess the value of semi-quantitative analysis of dynamic contrast enhanced magnetic resonance imaging (DCE-MRI) in distinguishing malignant lesions from benign lesions, and providing objective data. Materials and Methods: 95 mass lesions in 49 female patients were included. Time intensity curves ( TIC) were created for each lesion. Semiquantitative analysis was provided from the TIC curves using the postprocessing software. In the evaluation of qualitative assessment of kinetic curves, TICs were divided into three categories as progressive, plateau and wash-out, according to their behavior at mid-to-late stage. Results: The time-to-peak (Tpeak) values were significantly shorter for malignant lesions (p=0.01). No other semiquantitative parameter showed statistically significant differences between two groups. Curve type was significant in discriminating malignancy. Conclusions: The evaluation of TIC patterns and Tpeak are valuable for differentiation of benign and malignant lesions
