Brucellosis as a forgotten cause of epididymo-orchitis: a retrospective study of 34 patients

Özet

Giriş: Brucella özellikle endemik bölgelerde görülen insan ve hayvanları birlikte etkileyen, sistemik tutulumlu bir zoonozdur. Brucella bakterileri insanlara enfekte olmuş hayvanlardan ya da hayvan ürünlerinden en sık gastrointestinal sistem yoluyla geçmektedir. Brusellozlu hastaların %2-20’sinde epididimoorşit görülmektedir. Bildirimizde ülkemizde yayınlanmış en geniş brusella epididimoorşiti serisi sunulmaktadır. Materyal ve Metot: 2010-2012 yılları arasında Siirt Devlet Hastanesi ve Siirt Kadın Doğum ve Çocuk Hasta-nesinde Brusella epididimoorşiti tanısı konulup, tedavi edilen 34 adet hastanın dosyası retrospektif olarak ince-lendi. Hastaların tümünün demografik verileri, klinik ve laboratuar bulguları, tedavi, sonuç ve takip bilgileri çıkartıldı. Tüm hastaların tanısı skrotal şişlik ve ağrı, ateş, terleme gibi şikayetlerle hastaneye başvurma son-rası yapılan standart tüp aglutinasyon testleri (≥1/160 titre) ile konuldu. Hastalara Tetrasiklin 100 mg peroral günde iki defa ve Rifampisin 600mg/gün peroral olarak 6 hafta süreyle tedavi uygulandı. Hastalar 15. ve 45. günlerde kontrol edildi. Bulgular: Hastaların yaş ortalaması 29,2 (5-50) idi. Unilateral tutulum 30 hastada gözlenirken, bilateral tutulum 4 hastada mevcuttu. Tüm olgular medikal tedavi ile başarılı olarak tedavi edilmesine rağmen, 1 hastada nekrotizan orşite bağlı testiküler apse gelişmiş, medikal sağaltım sonrası ağrı ve şişliğin devam etmesi sonucu sol skrotal içerik testis ile birlikte çıkarıldı. Diğer 33 hastada medikal tedavi ile klinik belirtiler geçti. Sonuç: Özellikle hayvancılığın yaygın olduğu bölgelerde, sistemik semptomların da eşlik ettiği olgularda brusellozun epididimoorşit nedeni olabileceği unutulma-malı, hastalar buna yönelik olarak değerlendirilmeli ve tedavi edilmelidir.

Abstract

Introduction: Brucellosis, a zoonosis, is a multiorgan infectious disease that affects both humans and animals. Brucellosis is transmitted to humans from infected animals or their products mainly through gastrointestinal tract. Epididymo-orchitis is described in 2-20% of patients with brucellosis. We are reporting the largest series of epididymo-orchitis due to brucellosis. Material and Method: According to the records of Siirt Goverment Hospital and Siirt Woman Birth and Child Hospital between 2010-2012, 34 patients of epididymo-orchitis due to Brucellosis hospitalised and followed up were reviewed, retrospectively. All patients had scrotal swelling, fever, sweating and scrotal pain and were diagnosed with standart tube agglutination tests (the latter in a titer of 1/160). The patients were treated with a medication composed of tetracycline 100mg p.o b.i.d and rifampicin 600 mg/day p.o for 6 weeks, then were evaluated at the 15th and 45th days. Results: The mean age of patients was 29,2 (5-50). Thirty out of the 34 patients were found as unilateral and four of them as bilateral disease. In spite of medical therapy, one of 34 patients had a testicular abscess requiring left orchiectomy. Clinical signs and symtoms resolved in the other 33 patients with medical treatment. Conclusion: Brucellosis should be considered and treated as a cause of epididiymo-orchitis in patients who have systemic symptoms particularly those living in the endemic regions where cattle dealing is widespread.
