Deneysel inmemiş testis modelinde beta HCG tedavisinin fertilite indeksi üzerindeki etkisi

Özet
Kriptorşidizmde hormon tedavisiyle ilgili olarak yapılan farklı çalışmalarda, testisin ve germ hücrelerinin histolojik yapısına olumlu ya da olumsuz etkilerinin olabileceğine dair farklı sonuçlar bildirilmesi, bu tedavinin sorgulanmasına neden olmuştur. Biz de sıçanlar üzerinde deneysel kriptorşidizm oluşturarak, β-hCG tedavisinin fertilite indeksi (Fİ) açı-sından yan etkilerinin irdelendiği bir çalışma yaptık. Çalışmamız, 22 günlük, sağlıklı, 30 adet Spraque-Dawley cinsi erkek sıçan üzerinde yapıldı. Sıçanlar sham (kontrol), deneysel kriptorşidizm ve deneysel kriptorşidizm oluşturularak hormon tedavisi verilecek 3 gruba ayrıldı. Hormon tedavisi gruplarındaki sıçanlara deneysel inmemiş testis oluşturulduktan sonra-ki ilk günden itibaren 7 gün boyunca β-hCG uygulandı. Kriptorşidizmin ve β-hCG’nin etkilerini değerlendirmek için, inmemiş testis oluşturulduktan sonraki 30. günde orşiektomi yapıldı. Her bir sıçan için, 50 adet seminifer tübül ve içindeki spermatogoniumlar sayılarak Fİ hesaplandı. Tüm grupların Fİ değerleri birbirleriyle kıyaslandı. Sham grubunun fertilite indeksi, kriptorşidizm ve hormon grubundan anlamlı derecede yüksek bulunurken, hormon verilenlerde sadece kriptorşidizm yapılanlara göre sayısal olarak daha düşük saptanmakla birlikte bu fark istatistiksel olarak anlamlı değildi. Çalışmamızda β-hCG tedavisinin, sadece kriptorşidizme göre anlamlı derecede artmış bir olumsuz etkisi gösterilemedi. Bu konuda daha doğru bilgiler elde edilmesi için deneysel çalışmalardan çok, gerçek inmemiş testisli olgular içeren geniş klinik çalışmaların yapılması gereklidir
Abstract
Hormone treatment in cryptorchidism has been questioned because of the controversial reports suggesting either positive or negative effects on testis and germ cells. We performed a study to evaluate the side effects of β-hCG treatment in terms of fertility index (FI) in an experimental cryptorchidism model. Our study was carried out on 30 healthy, male, Sprague Dawley rats of twenty-two days old. These rats were divided into 3 groups such that sham, experimental cryptorchidism (EC) and experimental cryptorchidism treated with hormone groups (HT). HT group received β-hCG daily for the next 7 days after EC was performed. Orchidectomy was done on the 30th day after EC surgery in order to evaluate the effects of EC and β-hCG treatment. For each rat, 50 seminipherous tubuli and the spermatogonia inside them were counted to calculate FI. FI values were compared to each other among the 3 groups. SPSS for Windows 11.0 software was used for statistical analyses. Data were analyzed with Variance Analysis (ANOVA, Post-Hoc Scheffe test) and t test for paired samples. FI was higher in Sham group than the others, while it was smaller in HT than EC group, the former being statistically significant, the latter insignificant. We failed to show any significant additional negative effects of β-hCG treatment on the experimental cryptorchidism model, when compared with experimental cryptorchidism alone. In order to obtain more accurate data, it is essential to perform large clinical studies including patients with congenital cryptorchidism, as opposed to an EC model.
