Romatizmal Kalp Hastalıklarında Ortalama Trombosit Hacmi ve Trombosit Dağılım Genişliği

Özet
Amaç: Romatizmal kalp hastalığı (RKH), kardiyovasküler hastalıkların gelişmekte olan ülkelerde endemik seyreden önemli ve önlenebilir bir nedenidir. Bu çalışmanın amacı RKH’da ortalama trombosit hacmini (MPV) ve trombosit dağılım genişliğini (PDW) araştırmaktır. Materyal ve Metod: Bu çalışma Kayseri Sağlık Bilimleri Üniversitesi Çocuk Kardiyoloji Bölümü’nde, Şubat 2000-Şubat 2017 tarihleri arasında retrospektif olarak yedi yıllık süreçte izlenen 135 RKH’li çocuk hasta ile yaş ve cinsiyetleri benzer 40 sağlıklı çocuğu içermektedir. Bulgular: Çalışmaya alınan 135 olgunun yaş ortalaması 12,43±3,11 yıl, 70’i erkek (%51,8), 65’i kız (%48,2) idi. Kontrol grubunun yaşları ortalaması 12,96±2,55 yıl, 21’i erkek (%52,5)),19‘u kız (%47,5) olmak üzere 40 idi. Hastaların MPV değerleri RKH’lı grupta anlamlı derecede düşüktü (9,43±1,31 ve 10,21±1,25; p:0,003). Tersine PDW seviyeleri RKH’lı grupta anlamlı derecede yüksekti (15,70 (15,40-16,00) ve 12,25(10,40- 14,30); p: 0,001). MPV/PDW oranı RKH’lı grupta anlamlı derecede düşük saptandı (0,61(0,55-0,71) ve 0,87(0,79 0,93); p: 0,001). Sonuç: RKH sürecinde kontrol grubuyla kıyaslandığında MPV düşüklüğü ve PDW yüksekliği mevcutttur.
Abstract
Objective: Rheumatoid heart disease (RHD) is an importantand preventable cause of cardiovascular disease in developingcountries. The aim of this study was to investigate the meanplatelet volume (MPV) and platelet distribution width (PDW)in RHD.Material and Methods: This study was carried out in KayseriHealth Sciences University, Department of Pediatric Cardiology, with 135 children with RHD who were retrospectivelyobserved between February 2000 and February 2017 for sevenyears and 40 healthy children with similar age and sex.Results: The mean age of the 135 patients enrolled in the studywas 12,43 ± 3,11 years, 70 were males (51,8%) and 65 werefemales (48,2%). The mean age of the control group was 12,96± 2,55 years, 21,21 male (52,5%) and 19 female (47,5%). MPVvalues of the patients were significantly lower in the RHDgroup (9,43 ± 1,31 and 10,21 ± 1,25, p: 0,003).Conversely, the PDW levels were significantly higher in theRHD group (15,70 (15,40-16,00) and 12,25 (10,40-14,30), p:0,001). When MPV / PDW ratio was examined, it was found tobe significantly lower in the RHD group (0,61 (0,55-0,71) and0,87 (0,79-0,93), p: 0,001).Conclusion: MPV values were lower and PDW values were higher when compared to controls in RHD process.
