Derin Ven Trombozu Olan Hastaların Takibinde DDimer Testinin Önemi

Özet
Amaç: Derin ven trombozu (DVT) tedavi edilebilir bir hastalık olmasına rağmen, nüks etmeye kuvvetli bir eğilim gösterdiğinden, pulmoner emboli ve post-trombotik sendrom gibi ciddi komplikasyonlara yol açabilmektedir. Hastalığın tekrarlanmasından kaçınmak için, warfarin tedavisinin süresi uzatılabilir. Ne yazık ki, DVT hastalarında antikoagülan tedavinin optimal süresi belirsizdir. Antikoagülan tedavi tamamlandıktan bir ay sonra yapılan D-dimer testi hastalığın rekürrens riski hakkında fikir verebilir. Çalışmamızın amacı warfarin tedavisi tamamlandıktan sonra idiyopatik DVT rekürrenslerini değerlendirmede D-dimer testinin prediktif değerini araştırmaktır. Gereç ve Yöntem: Çalışmamız hastanemizde idiyopatik DVT tanısı konulan 70 hastayı kapsamaktadır. DVT tanısı Wells Skorlama sistemi ve D-dimer testiyle kondu ve venöz Doppler ultrasonografiyle kesinleştirildi. Tüm hastalar da warfarin kesildikten yaklaşık bir ay sonra D-dimer testi uygulandı ve 18 ay süreyle izlendiler. Bulgular: Hastalarımızın 36’sı erkek ve 34’ü kadındı, yaşları 32 ile 76 arasında değişmekteydi (ortalama 54.3±13.2 yıl). Ddimer testi 21 (%30) hastada pozitif bulundu. Venöz Doppler ultrasonografik incelemede D-dimer testi pozitif olan bu 21 olgunun 12’sinde (%57,4) rezidüel venöz obstruksiyon (RVO) tespit edilirken, D-dimer negatif olan 49 olgunun ise ancak 10’unda (%20,4) RVO tespit edildi. Hem D-dimer testi pozitif ve hem RVO olan 12 olguda, antikoagülan tedavi 6 ay süreyle uzatıldı. Antikoagülan tedavi süresi uzatılmayan geri kalan 58 hastanın 8’inde (%13,8) rekürrens gelişti. Rekürrens gelişen olguların birinde homozigot FVL polimorfizmi tespit edildi. Sonuç: DVT olan hastalarda warfarin tedavisi kesildikten bir ay sonra yapılan D-dimer testi rekürrens olaylarını tahmin etmede yararlı olabilir. D-dimer testi pozitif ve RVO olan hastalarda antikoagülan tedavinin tekrar başlanması faydalı olabilir. Bununla birlikte D-dimer seviyelerini etkileyen diğer klinik durumlarda göz önüne alınmalıdır.
Abstract
Objective: Although deep venous thrombosis (DVT) is a treatable disease, it has a strong tendency to recur leading to serious complications such as pulmonary embolism and postthrombotic syndrome. To avoid recurrences, the duration of warfarin treatment may be prolonged. Unfortunately, the optimal duration of anticoagulation treatment in patients with DVT is uncertain. D-dimer testing one month after the completion of anticoagulant therapy may give an idea about the risk of recurrence of disease. The aim of this study was to investigate the predictive value of D-dimer testing for recurrent events after withdrawal of warfarin therapy in patients with DVT. Materials and Methods: This study was carried out in our University Hospital and included 70 patients with unprovoked DVT. The diagnosis of DVT was based upon Wells scoring system and D-dimer testing and confirmed by venous Doppler ultrasonography. All patients underwent D-dimer testing approximately one month after warfarin withdrawal and they were followed for 18 months. Results: There were 36 male and 34 female patients ranging in age from 32 to 76 years, with a mean age of 54.3±13.2 years. Twenty one (30%) patients had positive D-dimer testing. While 10 (20.4%) of the 49 patients with negative Ddimer testing had residual venous obstruction (RVO), 12 (57.4%) of the 21 patients with positive D-dimer testing had RVO. In 12 patients with positive D-dimer testing and RVO, duration of coumadin therapy was prolonged for 6 months. In the remaining 58 patients, DVT recurred in 8 (%13.8) patients, of whom one was found to be homozygous FVL polymorphism. Conclusion: D-dimer testing should be useful to predict recurrent events one month after the discontinuing of warfarin therapy in patients with DVT. Resumption of anticoagulation therapy may be useful in patients with positive D-dimer testing and RVO. However, other clinical conditions affecting the D-dimer levels should be taken into consideration.
