Diferansiye tiroid kanseri tanılı hastalarda ablasyon ve re-ablasyon tedavisi başarısı

Özet

Amaç: Diferansiye tiroid kanseri tanıs ı alm ış ve I-131 ablasyon tedavisi verilmiş ancak ablasyon tedavisi ba ş arıs ız olan hastalarda, ikinci ve üçüncü ablasyon tedavilerinin ba ş arıs ı ara ş tır ılm ış t ır. Yöntem: Temmuz 2007-Eylül 2009 tarihleri aras ında kliniğ imize (Radyonüklid Tedavi Polikliniğ i’ne) ba ş vuran, diferansiye tiroid karsinomu tanıs ı alm ış , total/totale yakın tiroidektomi yapılm ış ve I-131 ablasyon tedavisi alan ve çalış ma süresi içinde en az 6. ay kontrollerine gelen tüm hastalar de ğ erlendirmeye alındı. Hasta dosyaları retrospektif olarak tarandı. Hastalar ın 6. ay I-131 TVT görüntülerinde tiroid lojunda rezidü saptanmayan hastalar ‘ablasyon ba ş ar ılı’, rezidü saptanan hastalar ise ‘ablasyon ba ş ar ıs ız’ olarak kabul edildi. Bulgular: Çalış mada diferansiye tiroid karsinom tanılı 191 hasta değ erlendirildi. 3 doz sonras ı ablasyon ba ş arılar ı s ıras ıyla %74.3, %75 ve %100 bulundu. Reablasyon dozu sonras ı tedavi ba ş ar ıs ı ile hastaya ve hastalığ a ait faktörler aras ında herhangi bir iliş ki saptanmadı. Tg ortalamas ı “ablasyon ba ş ar ıs ız” grupta, “ablasyon ba ş ar ılı” gruptan anlamlı olarak yüksek bulunmuş tur (p<0,005). Sonuç: İ kinci ablasyon dozu verilen olgularda beklenen yüksek tedavi ba ş arıs ı, stunning etkisine ba ğ lı olarak elde edilememiş olabilir. Ayr ıca Tg de ğ eri arttıkça reablasyon ba ş arıs ının düş tüğ ü gözlenmiş tir.

Abstract

Aim: To investigate the success of ablation and re- ablation therapy in patients with differential thyroid cancer who have failed with the first I-131 ablation therapy. Method: All of the patients with differential thyroid cancer who had undergone total or near-total thyroidectomy and who were referred for I-131 ablation therapy to the Nuclear Medicine Department between July 2007 and September 2009 were included. The patients had at least sixth months of follow-up. The patients’ records were evaluated retrospectively. The presence of residual thyroid activity on the 6th month diagnostic I-131 WBS image was accepted as “successful ablation” and the absence of residual thyroid activity was accepted as “unsuccessful ablation”. Results: 191 patients with differential thyroid cancer were assessed in this study. The overall success rate of the first, second and third ablation therapy was found as 74.3%, 75% and 100% respectively. There was no significant correlation between the patients' disease related factors and dosage of I-131 therapy. The average Tg value in group of “successful ablation” was found significantly higher than the group of “unsuccessful ablation” (p<0,005). Discussion: The expected higher rates in success of ablation therapy in patients with re-ablation cannot be obtained due to the effect of stunning. Furthermore, when Tg levels increased, it was observed that the success of ablation therapy decreased.
