Özet

Derin ven trombozunda rekürenslerin ve postfilebitik sendromun önlenmesiyle antikoagülan tedavinin süresi arasındaki ilişkiler tartışma konusu olmuştur. Amacımız alt ekstremite derin ven trombozunda kısa süreli antikoagülan tedavinin uzun süreli tedavi kadar etkili olup olmayacağını tesbit etmektir. Alt ekstremite derin ven trombozu tanısı konulan 84 hasta çalışma kapsamına alındı. Tanıda venografi ve/veya dubleks ultrasonografi kullanıldı. 43 hastada (grup A) Uzun süreli antikoagülan tedavi uygulandı. Önce heparinizasyon yapılarak 4., 5. ve 6.günler coumadin tedavisi eklendi. 7. Günden itibaren heparin kesilerek yalnız coumadin ile tedaviye 3 ay devam edildi. 41 hastada ise kısa süreli antikoagülan tedavi uygulandı. Yine heparin ile tedaviye başlanarak 2.,3. ve 4. Günler tedaviye coumadin eklendi. 5. Günden itibaren yalnız coumadin ile tedaviye 45 gün devam edildi. İlk 3 ayda 45 gün aralıklarla sonrada 3 ay aralıklarla dubleks Doppler ultrasonografi incelemesi yapıldı.Her iki grup arasında risk faktörleri, lokalizasyon ve ortalama yaş bakımından anlamlı fark yoktu. A grubundaki hastaların yaşı 27 ile 73 arasında değişmekte ortalama yaş 50 ± 4,5 idi. B grubundakilerin ise 32 ile 80 arasında değişmekte ortalama yaş ise 52 ± 3 idi. A grubunda l olguda pulmoner emboli, 2 olguda aşırı kanama; B grubunda l olguda pulmoner emboli, l olguda aşırı kanama oldu. A grubunda 18 (%41,8), B grubundaysa 19 (%46) olguda postflebitik sekel gelişti. A Grubunda 9 (%22), B Grubunda 10 (%23) hastada rekürens gelişti. 45 günlük kısa süreli antikoagülasyon, uzun süreli antikoagülasyon kadar etkili olmakta, reflü, rekürens ve kanama gibi komplikasyonların gelişmesi bakımından her iki grup arasında anlamlı bir fark yoktur.

Abstract

The relations among the duration of anticoagulation, incidence of postphlebitic syndrome and recurrence of disease in patients with lower limb deep venous thrombosis has been debated. In this study, we compared the results of treatment in patients with DVT between long and short-term anticoagulation. This is a prospective study of 84 patients who were hospitalized for symptomatic unilateral DVT of the lower limb. Diagnosis was made using duplex ultrasound and/or venography. They were divided into two groups (group A and B). Patients in group A received long-term anticoagulation treatment, which included initial heparin followed by coumadin on days 4-6 and heparin was discontinued on day 7. Coumadin was continued for 3 months. Patients in this Group B received short-term anticoagulant treatment, which included the same initial heparin therapy followed by warfarin on days 2-4 and heparin was discontinued on day 5. Warfarin treatment was continued for only 45 days. Duplex ultrasound follow-up was done at 45 days intervals, and every 3 months after periods. In both groups, heparinization was adjusted as to the activated partial thromboplastin time at 1,5 to 2-fold of control; warfarin treatment was maintained with an INR value of 2 to 3 or a prothrombin time of 1,5 to 2 fold of the control. There were no differences between The Group A and The Group B patients with respect to demographic data, risk factors and location of DVT. In group A patients, the age range was 27-73 years with an average of 50 ± 4,5 years. In The Group B patients, this was 32-80 years with an average of 52 ± 3 years. There were 3 major complications in The Group A patients; l PE and 2 significant hemorrhages. In The Group B patients, there were 2 major complications; 1 PE and 1 significant hemorrhage. 18 patients (41,8%) in group A and 19 patients (46%6) in The Group B had postphlebitic changes. 9 patients in group A and 10 patients in The Group B had one or more recurrent venous thromboembolic events. We concluded that short-term anticoagulant therapy is as effective as long-term anticoagulant therapy and there are no significant differences between long and short-term anticoagulant therapies in respect for postphlebitic changes, recurrences and hemorrhage.
