Prematüre Retinopatisinde Retinopati Şiddetinin Prematüre Evresine Göre Değerlendirilmesi

Özet

Amaç: Bu çalışmada amaç, prematüre retinopati (PR) tarama sonuçlarını analiz ederek, prematüre evresinin retinopati şiddeti ile ilişkisini değerlendirmektir. Gereç ve Yöntem: Mardin Devlet Hastanesi’nde, Haziran 2017- Eylül 2018’de kliniğimize başvuran prematür bebeklerden 431 olgunun 862 gözüne ait veriler retrospektif olarak değerlendirildi. Olgular gestasyonel yaş ve doğum ağırlıklarına göre sınıflandırıldı. Olgular ileri derece, orta derece ve geç prematüre diye ayrıldı. Çoğul doğumlar kaydedildi. Retinopati şiddeti ile prematüre derecesi ve çoğul doğum ilişkisi değerlendirildi. Bulgular: Olguların 234’ü (%54,3) kız, 197’si (%45,7) erkek olup, doğum haftaları 23-36 hafta (31,9±2,77), doğum ağırlıkları ise 510-3500 gr (1746±476) arasındaydı. Herhangi bir evrede PR olguların 137’sinde (%31,8) olup, 294 (%68,2) olguda PR bulgusu saptanmadı. Evre 1 PR 80 olguda (%58,4) , Evre 2 PR 42 olguda (%30,7), Evre 3 PR 13 olguda (%9,5), Evre 4 PR ise 2 olguda (%1,5) tespit edildi. Herhangi bir evrede PR bulgusu olan olguların ortalama doğum ağırlığı ve doğum haftası istatistiksel olarak anlamlı daha küçük idi (p 32 haftalık infantlarda bu oran %35,1 olarak görüldü. Doğum haftası >32 hafta olan 6 (%4,32) olguda Evre 3 ve Evre 4 PR saptandı. Çoğul doğum ile PR arasında anlamlı ilişki bulunamadı. (p=0,908) Sonuç: Orta derece ve geç prematürlerde de ileri evre PR olabileceği her zaman göz önünde bulundurulmalıdır. Prematüre retinopatisinde tarama, tüm prematürlerde dikkatle yapılmalıdır.

Abstract

Objective: The aim of this study is to analyze the relationship between the stage of prematurity and retinopathy severity by analyzing the results of premature retinopathy (PR) screening. Material and Method: In Mardin state Hospital 862 eyes of 431 cases who were admitted between June 2017 and September 2018 were evaluated retrospectively. Cases were classified according to gestational ages and birth weights. The cases were divided into advanced, intermediate and late premature. Multiple births were recorded. The relationship between the severity of retinopathy and the degree of prematurity and multiple births was evaluated. Results: This study included 862 eyes of 431 infants, 234 (54.3%) of the cases were female and 197 (45.7%) were male. Birth weeks were 23 to 36 weeks (31.9 ± 2.77), and birth weights were 510-3500 gr (1746 ± 476). PR was detected in 137 (31.8%) of the cases at any stage, and no sign of PR was found in 294 (68.2%) cases. Stage 1 PR was detected in 80 cases (58.4%), stage 2 PR in 42 cases (30.7%), stage 3 PR in 13 cases (9.5%), and stage 4 PR in 2 cases (1.5%). The mean birth weight and gestational week were statistically significantly lower in patients with PR at any stage (p 32 weeks of age. Stage 3 and Stage 4 PR were detected in 6 (4,32%) cases with a gestational age of >32 weeks. No significant relationship was found between multiple births and PR. (P = 0.908) Conclusion: In premeture retinopathy, screening should be performed carefully in all prameture infants. It should always be considered that there may be advanced stage PR in moderate and late premature.
