Severe hypotension due to a single dose of quetiapine in an elderly patient: A case report

Özet

Hipotansiyon ba ş dönmesi, halsizlik ve senkop gibi belirtilerle seyreder. Sistolik kan bas ınc ının 80-90 mmHgnın, diastolik kan bas ınc ının da 50-60 mmHgnın altında olmas ı ş eklinde tanımlanabilir. Ya ş lı bireylerde hipotansiyon düş me ve kırıklara neden olabildiğ i için tehlike oluş turmaktadır. Ketiapin; ekstrapiramidal semptomlara yol açmamas ı, plazma prolaktinini yükseltmemesi ve antikolinerjik yan etkilere yol açmamas ı nedeniyle ya ş lılarda kullanım ı öne çıkan atipik antipsikotik ilaçlardand ır. Bu yaz ıda hipertansiyon öyküsü olan ve antihipertansif ilaç kullanan ancak tek doz 100 mg ketiapin verildikten sonra ba ş dönmesi ve göz kararmas ı ş ikâyetleri olup a ğ ırla ş arak yo ğ un bakım ko ş ullar ında stabilize edilen bir olgu sunulmuş tur.

Abstract

Hypotension has symptoms such as dizziness, weakness and syncope. Hypotension, defined as below systolic blood pressure below 80-90 mmHg, and diastolic blood pressure below 50-60 mmHg. Hypotension is dangerous causing falls and fractures in older people. Quetiapine is a atypical antipsychotic drug more used in elderly patients because it does not cause extrapyramidal symptoms and anticholinergic side effects and does not lead to increased plasma levels of prolactin. In this article, we introduced a case report with a history of hypotension and using antihypertensive medicine, however, after a single dose of 100 mg quetiapine, complaints of dizziness and blackouts that have been stabilized under intensive care conditions.
