Çocukluk çağında adenotonsillektominin büyüme ve geliş meye olan etkisi

Özet
Amaç: Çalış mam ızda kronik tonsillit nedeniyle tonsillektomi operasyonu uygulanm ış olan 100 hastada operasyonun büyüme ve geliş meye olan etkisini ara ş t ırmay ı amaçladık. Gereç ve Yöntem: Çalış mam ız, Haziran 2004 ile Şubat 2005 tarihleri aras ında, tekrarlayan tonsil enfeksiyonları ve havayolu obstrüksiyonu nedeniyle, hastanemiz kulak burun bo ğ az kliniğ ine ba ş vuran ve tonsillektomi endikasyonu konan 100 hastada prospektif olarak ele alındı. Bu amaçla operasyonun büyüme ve geliş meye olan etkisini irdelemek amaçlı, preoperatif ve postoperatif 1.- 3. ayda boy, kilo takibi ve persantil da ğ ılım ına bakıldı. Elde edilen veriler say ısal ş ekilde kodlanarak SPSS sürüm 10,0 ile de ğ erlendirildi. p<0.05 değ erleri istatistiksel olarak anlamlı kabul edildi. Çalış ma, İ zmir Dr. Behçet Uz Çocuk Sa ğ lığ ı Hastalıklar ı ve Cerrahisi E ğ itim ve Ara ş tırma Hastanesi etik kurulunun 22.12,2004 tarih ve 11 say ılı onay kararı alınarak yürütüldü. Bulgular: Olgular ın operasyon öncesi ve operasyon sonras ı 1-3. ayda vücut a ğ ırlıklar ı ölçülerek kar ş ıla ş tır ıld ı. Postoperatif 1. ve 3. Ayda kilo art ış ı, preoperatif döneme göre istatistiksel olarak anlamlı idi (p< 0,05). Olgular ın operasyon öncesi ve operasyon sonras ı 1.-3. aylarda boylar ı ölçülerek karş ıla ş t ır ıldı. Postoperatif 1. ve 3. ayda boy uzamas ının istatistiksel olarak anlamlı arttığ ı saptandı (p<0,05). Sonuç: Kronik tonsillit nedeni ile çocuklarda büyüme ve geliş menin negatif yönde etkilendiğ i, tonsillektomi sonras ı hastalar ın kilo ve boy ölçümlerinin artt ığ ı, büyüme eğ risinde üst e ğ rilere geçtikleri saptandı.
Abstract
The aim of our study was to evaluate the effect of tonsillectomy operation on growth and development of children. Method: In this prospective study were included 100 cases who underwent tonsillectomy operation because of recurrent tonsil infection and airway obstruction between June 2004 and February 2005. Izmir Dr. BehçetUz Children's Health Diseases and Surgery Training and Research Hospital ethics committee approval was obtained. Body height, weight and percentile distributions of cases were recorded preoperatively and at postoperative 1 - 3 months to examine the impact of growth and development of the operation. SPSS 10 was used for statistical analysis. p <0.05 was considered statistically significant. Preoperative, postoperative first and third months body weights were compared with each other as well as body height. Results: Weight gain at postoperative first and third months were significant compared to preoperative period (p< 0,05). Also extension of height increased at postoperative first and third months (p<0,05). Conclusion: Chronic tonsillitis negatively affects the growth and development of children. Weight and height measurements of patients increase and the growth curves pass through the upper curve after tonsillectomy.
