Prehipertansif Hastalarda Yaşam Tarzı Değişikliklerin Ekokardiyografik ve Biyokimyasal Parametreler Üzerine Etkisi

Özet
Amaç: Son yapılan çalışmalara göre hipertansiyonun habercisi olan prehipertansiyonun prevelansının ülkemizde ve dünyada oldukça fazla olduğu tespit edilmiş olup gittikçe artacağı tahmin edilmektedir. Araştırmamızda, DASH diyeti ve egzersiz tedavisinin biyokimyasal, klinik ve ön planda diyastolik olmak üzere tüm ekokardiyografik parametrelere etkisinin belirlenmesi amaçlandı. Gereç ve Yöntem: Ek bir hastalığı olmayan, 20- 40 yaş arası prehipertansif (kan basıncı 120/80- 140/90 mmHg; 13E, 5K) ve normotansif (kan basıncı 120/80 mmHg altında; 6E, 7K) iki gruba 6 ay süre ile DASH diyeti ve egzersizverilerek tedavi öncesi ve sonrası klinik, laboratuvar değerlendirmesi ilediyastolik disfonksiyon için ekokardiyografik inceleme yapılarak, tedavinin bu parametreler üzerine etkisi ölçüldü. Bulgular: Prehipertansif grubun vücut ağırlığı, GFR, homosistein ve OGTT açısından normotansif gruptan farklı olduğu; prehipertansif grubun DASH diyeti ve egzersiz tedavisine sistolik ve diyastolik kan basıncı, insülin düzeyi, insülin direnci, GFR ve homosistein açısından normotansif gruba göre daha iyi cevap verdiği; çalışma başında 17 prehipertansif hastanın 6'sında (%35.29) diyastolik disfonksiyon olduğu ve tedaviden sonra bunların 4 (%66.6)'ünün düzeldiği tespit edildi. Sonuç: DASH diyeti ve egzersiz tedavisi ile prehipertansif grupta yüksek olan DKB ve SKB'ı, insülin ve homosisteinin düzeyleri ve insülin direnci etkin olarak düzelmiştir. Prehipertansiflerde DASH diyeti ve egzersizin diyastolik disfonksiyon üzerine ne kadar etki ettiğini değerlendirmek için vaka sayısının fazla olduğu daha büyük bir çalışmaya ihtiyaç vardır.
Abstract
Objective: Recent studies show that the prevalence of prehypertension as a predictive of hypertension is common in both our country and worldwide and will increase. We aimed to determine the effects of DASH diet and exercise treatment on clinical, biochemical and predominantly diastolic echocardiografic parameters in prehypertensive patients Materials and Methods: Subjects were 20 to 40 years old without an additional disease and grouped into two: Prehypertensive group included subjects with blood pressure of 120 to 139 mmHg/ 80 to 89 mmHg and normtensive groupe included subjects with blood pressure < 120/80 mmHg. They were treated with DASH diet and exercise for 6 months. After treatment, we evaluated the clinical, biochemical and echocardiografic parameters in prehypertensive and normotensive groups again. Results: It has been shown that the basal weight, GFR, OGTT and homocysteine in prehypertensive group were different from normotensive group, prehypertensive group responded to diet and exercise better than the normotensive group in terms of systolic and diastolic blood pressure, insülin levels, insulin resistance, GFR and homocysteine. In our study there were 6/17 (35.29%) patients with diastolic dysfunction in the prehypertensive group, 4 (66.6%) of whom recovered after treatment. Conclusions: Significant improvements in systolic and diastolic blood pressure, insulin, insulin resistance and homocysteine were found with DASH diet and exercise in prehypertensive subjects. Further studies with large number of participants are needed to determine the effects of DASH diet and exercise on diastolic dysfunction in prehypertensive patients.
