İskemik Arteriyel İnmede Protrombin G20210A ve Faktör V Leiden Mutasyonları

Özet
Amaç: İskemik inme tüm dünyada önemli bir halk sağlığı sorunu olmaya devam etmektedir. Faktör V Leiden ve protrombin G20210A gibi bazı genetik mutasyonların iskemik venöz inme riskine katkıda bulunduğu bilinmektedir. Bununla birlikte, bu genetik mutasyonların arteriyel inme riskindeki rolü hala tartışmalıdır. Bu çalışmanın amacı, faktör V Leiden ve protrombin G20210A gibi genetik mutasyonlar ile arteriyel iskemik inme riski arasındaki olası ilişkiyi araştırmaktır. Gereç ve Yöntem: İskemik arteriyel inmeli 63 hasta ve 25 yaş ve cinsiyet yönünden benzer kontrol olgusu çalışmaya alındı. Bulgular: Protrombin G20210 mutasyonu, faktör V Leiden mutasyonu, hipertansiyon, hiperlipidemi ve hiperhomosisteinemi, kontrol grubuna kıyasla hasta grubunda belirgin olarak daha sık görüldü. Çok değişkenli analiz ile protrombin G20210A mutasyonu, iskemik arteriyel inme için bağımsız bir risk faktörü olarak bulundu. Faktör V Leiden mutasyonu ise iskemik arteriyel inme grubunda daha sık görülmesine rağmen, bağımsız bir risk faktörü değildi. Sonuç: Faktör V Leiden ve özellikle protrombin G20210A gibi genetik mutasyonlar inme için genetik risk faktörleri olarak düşünülmelidir. Arteriyel inme vakalarında ve inme için riskli gruplarda bu mutasyonları araştırmak, riski değerlendirmek ve hastalığı yönetmek için önemli olabilir.
Abstract
Objective: Ischemic stroke remains an important public health problem all over world. It is well known that, some type of genetic mutations such as factor V Leiden and prothrombin G20210A contribute to the risk of ischemic venous stroke. However, the role of those genetic mutations in the risk of arteriel stroke is still controversial. The aim of this study is to investigate the possible association between the genetic mutations such as factor V Leiden and prothrombin G20210A and the risk of arterial ischemic stroke. Material and Methods: Sixty-three patient with ischemic arterial stroke and 25 age and sex matched controls were included in the study. Results: Prothrombin G20210A mutation, factor V Leiden mutation, hypertention, hyperlipidemia and hyperhomocysteinemia have been found significantly more frequent in patients’ group compared to the controls. With the multivariate analysis prothrombin G20210 mutation was found as an independent risk factor for ischemic arterial stroke. Factor V Leiden mutation was not an independent risk factor although it was seen more frequent in the ischemic arteriel stroke group. Conclusions: Genetic mutations such as factor V Leiden and especially prothrombin G20210A should be considered as genetic risk factors for stroke. Exploring those mutations in the arteriel stroke cases and the high risk groups for stroke might be important to assess risk and manage the disease.
