Özet

Amaç: Özefagus kanserinin tedavisinde cerrahi hala en önemli kısmı oluşturmaktadır. Cerrahi, konvansiyonel yöntemler ile yapılabildiği gibi minimal invaziv yöntemlerle de yapılabilinmektedir. Bu çalışmada minimal invaziv McKeown özefajektomi yapılan olgularda azigos veninin polimer klips ile kapatıldığı olguları literatür eşliğinde tartışmayı amaçladık. Gereç ve Yöntem: Mayıs 2014-Mayıs 2016 yılları arasında merkezimizde özefagus kanseri nedeniyle minimal invaziv McKeown özefajektomi yapılan 13 olgu çalışmaya alındı. Transhiyatal özefajektomi, torakotomi ve azigos venin sütür ile kapatıldığı özefajektomi olguları çalışma dışı bırakıldı. Bulgular: Olguların yaş ortalaması 53,8 olup, 10 olgu kadın ve 3 olgu erkek idi. Tümör lokalizasyonu 10 olguda orta, 3 olguda alt özefagusta idi. Olguların 11’i neoadjuvant kemoradyoterapi almış iken 2 olgu almamıştı. On olguya torakoskopik-laparoskopik özefajektomi yapıldı, 3 olguya da torakotomik-laparoskopik özefajektomi yapıldı. Bütün olgularda azigos veni polimer klips ile kapatıldı ve özefagogastrostomi servikal bölgede yapıldı. Ortalama ameliyat süresi 210 dk idi. Ortalama hastanede kalış süresi 12,9 gün idi. Dört olguda patolojik tam yanıt vardı. Çıkarılan ortalama lenf nodu sayısı 9,6 idi. İki olguda komplikasyon olarak şilotoraks gelişti ve bunlardan birisi 26. günde kaybedildi. Hiçbir olguda azigos venine bağlı herhangi bir komplikasyon ve kanama yaşanmadı. Tartışma: Özefajektomi de önemli damarsal yapılardan birisi olan azigos veninin polimer klips ile kapatılmasının, maliyet ve kullanım kolaylığı açısından üstünlüğü olup güvenle kullanılabileceğini düşünmekteyiz.

Abstract

Objective: Surgery is still the most important part of the treatment of esophageal cancer. Surgery can be performed with conventional methods, and also with minimally invasive methods. In this study, we aimed to discuss the cases that azygos vein was closed with polymer clip in which the cases underwent minimally invasive McKeown esophagectomy. Material and Method: Between May 2014 and May 2016, 13 cases who underwent minimally invasive McKeown esophagectomy due to esophageal cancer in our center were included in the study. The cases that Transhiatal esophagectomy, thoracotomy and the case that azygos vein closed with suture were excluded from the study. Results: The mean age was 53.8 years, 10 cases were female and 3 cases were male. Tumor localization was middle esophagus in 10 cases and lower esophagus in 3 cases. While neoadjuvant chemoradiotherapy was received by 11 cases, 2 cases didn’t receive it. 10 patients underwent thoracoscopiclaparoscopic oesophagectomy, and 3 patients underwent thoracotomic-laparoscopic oesophyectomy. In all cases, azygos vein was closed with polymer clip and esophagogastrostomy was performed in the cervical region. The mean operative time was 210 min. The mean hospital stay was 12.9 days. 4 patients had pathological complete response. The mean number of lymph nodes removed was 9.6. In 2 cases, chylothorax developed as a complication and one of them died on the 26th day. None of the cases had any complications and bleeding due to azygos vein. Discussions: Closure of the azygos vein, one of the important vascular structures in esophagectomy, with polymer clips has an advantage in terms of cost and ease of use. Therefore we believe that it can be used safely.
