Özet

Amaç: Çalışmamızda dil lezyonu ile gelen hastaların demografik, klinik ve histopatolojik özellikleri retrospektif analiz edilip güncel literatürle karşılaştırıldı Gereç ve Yöntem: Bu çalışma, Ocak 2013-Aralık 2018 tarihleri arasında KBB kliniğimizde yapılmıştır. Dilinde lezyonu olan olguların klinik görünümleriyle birlikte biyopsiye dayalı olarak bu lezyonların histopatolojik bulguları kayıt edildi. Kaydedilen veriler yaş, cinsiyet, benign ve malign patolojilerin sıklığı, dildeki lezyonların lokalizasyonu, morfolojik görünümleri ve histopatolojilerine göre retrospektif analizleri yapılarak değerlendirildi. Bulgular: Çalışmaya dil lezyonu olan 261 hasta dahil edilmiştir. Hastaların 139’u kadın (%53,25) ve 122’si erkek (%46.75) olup biyopsi sonuçlarına göre 220’si (%84.29) benign, 41’i (%15.70) malign lezyon olarak saptanmıştır. Benign lezyonların 118’i kadın (%84,89) ve 102’si erkek (%83.60) olurken Malign lezyonlarda ise 21’i kadın (%15,10) ve 20’si erkek (%16.39) idi. Hastaların yaş ortalaması 52,45 olarak bulunmuştur. Benign patolojilerin en sık görüldüğü yaş grubu 40-59 yaş aralığında %40.6 oranında olup malign lezyonlar açısından da 60-79 yaş aralığında %34.5 olarak tespit edilmiştir. Benign lezyonların histopatolojilerine göre sıklık sırası; %20.9 ile akantoz/parakeratoz, %20.5 ile fibroepitelyal polip ve %20 ile papillom olduğu izlendi. Malign lezyonlardaki sıklık dağılımı ise; en sık %80.5 ile skuamöz hücreli karsinom olurken bunu %7.3 ile diffüz büyük B hücreli lenfoma izlemiştir. Bu lezyonların lokalizasyonlarına göre dağılımları ise; %34,8 ile dil lateral kenarı ve %34 ile dilin dorsal yüzeyi olmuştur. Tartışma: Klinik pratiğimizde dilinde uzun süreli lezyon ile başvuran hastaların klinik muayenesi, lezyonun görünümü ve histopatolojik analizi, lezyonun tipini kategorize etmeye yardımcı olur. Çalışma sonuçlarımıza göre dil lezyonlarının çoğu benign lezyonlar olup ancak dildeki vejetan, lökoplazik ve ülseratif görünümde olan bir lezyonun malign patolojinin ön bulgusu olabileceği akılda tutulmalıdır. Çünkü ağız boşluğu kanserlerinin dudaktan sonra en sık görüldüğü bölge dil olup hızlı metastaz yapması açısından erken tanı ve tedavisinin hastanın prognozuna önemli katkı sağlayacağını düşünüyoruz.

Abstract

Introduction: In our study, demographic, clinical appearance and histopathological results of patients with suspicious tongue lesions were analyzed retrospectively and compared with the current literature. Materials and Methods: This study was conducted in our Otorhinolaryngology clinic between January 2013 and December 2018. Histopathological findings of these lesions were recorded based on the biopsy of the patients with suspicious lesion in the tongue. The recorded data were evaluated according to age, gender, frequency of benign and malignant pathologies, localization of tongue lesions, morphological appearance and histopathology. Results: A total of 261 patients who presented to our clinic with the complaint of lesion were included in the study. Of these patients, 139 were female (53.25%) and 122 were male (46.75%). According to the biopsy results of the patients, 220 (84.29%) were benign and 41 (15.70%) were malignant lesions. Of the benign lesions, 118 were female (84.89%) and 102 (83.60%) were male. The mean age of patients was 52.45 years. The age group with the most common benign pathologies is 40.6% in the 40-59 age range. In terms of malignant lesions, it was found to be 34.5% in the age range of 60-79. According to the histopathology of benign lesions, the frequency order; 20.8% had acanthosis/parakeratosis, 20.5% fibroepithelial polyp and 20% papilloma. The frequency distribution in malignant lesions is; most commonly, squamous cell carcinoma was present in 80.5% of the cases, followed by diffuse large B-cell lymphoma with a rate of 7.3%. The distribution of these lesions according to their localizations; was lateral border of the tongue with 34.8% and dorsum of the tongue with 34%. Discussion: In daily practice, the examination of patients presenting with a long-term lesion, the appearance of the lesion, and the histopathological analysis help to categorize the type of lesion. According to our results, most of the tongue lesions are benign lesions but it should be kept in mind that a lesion with vegetative, leukoplakia and ulcerative appearance in the tongue may be a preliminary finding of malignant pathology, Because of the oral cavity cancer, tongue is the most common site after lip. We believe that early diagnosis and treatment of tongue cancers will contribute significantly to the prognosis of patients.
