Prostat Kanseri Tanısında Multiparametrik Manyetik Rezonans Görüntülemenin Önemi

Özet
Amaç: Transrektal ultrasonun (TRUS) prostat kanseri (PK) açısından şüpheli alanları saptamadaki yetersizliğinden dolayı son yıllarda multiparametrik manyetik rezonans (MPMR) görüntüleme eşliğinde prostat biyopsisi giderek yaygınlaşmaktadır. Bu çalışmada kliniğimizde uygulanılan MPMR kılavuzluğunda yapılan kognitif TRUS biyopsi yönteminin PK saptamadaki başarısını değerlendirmeyi amaçladık. Gereç ve Yöntem: Ağustos 2013-Mart 2015 tarihleri arasında Ankara Üniversitesi Tıp Fakültesi Üroloji PK şüphesi [Prostat spesifik antijen (PSA) değeri >=2,5 ng/ml ve/veya şüpheli parmakla rektal muayene (PRM) bulguları nedeniyle] MPMR kılavuzluğunda kognitif TRUS biyopsi yapılan 87 hastanın verileri incelendi. Hastalar MPMR'de şüpheli odak izlenen (51 olgu) ve izlenmeyen (36 olgu) olmak üzere iki gruba ayrıldı. Tüm olguların yaş, PSA düzeyi, PRM bulguları, önceki biyopsi öyküleri, tüm biyopsi numunelerinin patolojik sonuçları ve histopatolojik değerlendirme sonucu PK gelen numunelerin Gleason dereceleri kaydedildi. Bulgular: Olgularımızın ortalama yaşı 61±13,1, ortalama PSA değeri 6,55±3,64 ng/ml, ortalama prostat boyutu 52±25 gr idi. Olguların 79'unda yüksek PSA, 8'inde ise şüpheli PRM bulguları olması nedeniyle prostat biyopsisi planlanmıştır. Yetmiş iki olguya ilk kez biyopsi yapılırken, 15 olgunun ise daha önceden biyopsi öyküsünün olduğu öğrenildi. Yirmi altı (%29,9) olguda PK saptandı. PK saptanan 26 olgunun 23'ünde MPMR'deki şüpheli odaklardan alınan biyopsi numunelerinde, geriye kalan 3 olguda ise MPMR'de izlenen şüpheli odaklardan alınan biyopsi numunelerinde PK saptanmayıp sadece sistematik 12 odaklı biyopsi numunelerinde PK saptandığı belirlendi. MPMR incelemesinde şüpheli odak izlenmeyen 36 hastada yalnızca sistematik 12 odak biyopsi yapıldı ve hiçbirinde PK izlenmedi. MPMR kılavuzluğunda yapılan kognitif TRUS biyopsinin PK'yi saptamadaki duyarlılığı %100, özgüllüğü %59, negatif prediktif değeri %100, pozitif prediktif değeri %51 ve doğruluk oranının %71 olduğu belirlendi. Sonuç: İlk biyopsisinde PK saptanmayan ancak PK şüphesi devam eden ve tekrar biyopsi planlanan olgular başta olmak üzere tüm PK şüphesi taşıyan olguların biyopsi öncesi MPMR ile değerlendirilmesi önerilebilir.
Abstract
Objective: In recent years, prostate biopsy under multiparametric magnetic resonance (MPMR) imaging gradually increasing due to inability of transrectal ultrasonography (TRUS) for the detection of suspicious areas about prostate cancer (PC). In this study, we aimed to evaluate the success of MPMRI-guided TRUS cognitive biopsy on the detection of PC.Materials and Methods: We evaluated the data of 87 patients who underwent MPMRI-guided TRUS cognitive biopsy due to suspicion of PC [due to >=2,5 ng/mL prostate specific antigen (PSA) value and/or abnormal digital rectal examination] at Ankara University School of Medicine, Department of Urology, between August 2013 and March 2015. All patients were divided into two groups as positive (51 patients) and negative (36 patients) suspicious area on MPMRI. Data of age, PSA level, DRE findings, previous biopsy history, pathologic results and gleason score (in whom PC was detected) were recorded. Results: The mean age, the mean PSA value and the mean prostate volume of our cases were 61±13.1, 6.55±3.64 ng/mL and 52±25 g, respectively. We planned prostate biopsy due to high PSA value in 79 of all cases and suspicious DRM in 8 of all cases. First biopsy was performed in 72 of all cases whereas 15 patients underwent second biopsy. PC was detected in 26 (29.9%) patients. PC determined from suspicious locus on MPMRI in 23 of 26 patients and from only systematic 12 core biopsy in the remaining three cases. Only systematic 12 core biopsies were performed in 36 patients who had any suspicious locus on MPMRI, and PC was detected in none of these patients. Sensitivity, specificity, negative predictive value, positive predictive value and accuracy rate of MPMRIguided TRUS cognitive biopsy on detection of PC were 100%, 59%, 100%, 51% and 71%, respectively. Conclusion: Pre-biopsy MPMRI may be recommended for evaluation of PC in all suspicious cases, especially in patients who were planned rebiopsy for continuing of PC suspicion despite PC was not detected at initial biopsy
