Apre-pilot study of evaluation of changes in residual urine volume, detrusor pressure and bladder capacity by three different surgeries in symptom scores of prostati c patients and: review of the literature

Özet
Amaç: Prosatat volumleri aç ı s ı ndan homojen olmayan üç grup hastada, prostat ameliyat ı öncesine göre, ameliyattan alt ı ay sonraki erken dönemde prostat semptom skorlar ı nda, rezidüel idrar volumunde, Pdetristol Q max ve mesane maksimal kapasitesindeki de ğ i ş imi göstermek Gereç ve yöntem: Bas ı nç ak ı m çal ı ş mas ı yla tam mesane ç ı k ı m obstruksiyonu gösterilmi ş 75 AÜSS (alt üriner sistem semptomlu) hasta çal ı ş maya al ı nd ı . Bu ön çal ı ş maya, kontrole gelen 50 hasta dahil edilmi ş tir. 50 hastan ı n 15ine TV-P, 15ine TUE-P ve 20sine TUR-P uyguland ı . Tüm hastalar ı n operasyon öncesi ve sonras ı nda semptom skorlamas ı , Qmax, PdetQmax, maksimum mesane kapasitesi ve rezidüel idrar ölçümü yap ı ld ı . İ statistiksel analizde Paired t- test ve Kruskal-Wallis analizi kullan ı ld ı , p<0.05 anlaml ı kabul edildi. Bulgular: TVP, TUEP ve TURP gruplar ı için s ı ras ı yla ya ş ortalamalar ı 68.6, 61.3 ve 66.6, preoperatif ortalama pros tat volümleri 79.1 gr, 39.75 gr ve 54.8 gr idi. Gruplar operasyon öncesi ve sonras ı nda semptom skorlar ı nda, rezidüel idrar volumunde azalma, Pdetristol Q max ve mesane maksimal kapasitesi yönünden de ğ erlendirildi. TVP ve TURP grubundaki hastal arda bu parametrelerde anlaml ı de ğ i ş im saptanm ı ş t ı r. Öte yandan TUEP grubundaki hastalarda, sadece mesane maksimal kapasitesi yönünden anlaml ı de ğ i ş im izlenmemi ş tir. Ancak semptom skorlar ı nda, rezidüel idrar volumunde azalma, Pdetristol Q max anlaml ı de ğ i ş im izlenmi ş tir. Sonuç: Farkl ı prostat volumüne ra ğ men TVP ve TURP uygulanan hasta gruplar ı nda prostat semptom skorlar ı nda, rezidüel idrar volumunde azalma, Pdetristol Q max ve me sane maksimal kapasitesi yönünden hemen hemen benzer anlaml ı sonuç izlemi ş ken seçilmi ş hasta gruplar ı nda da mesane maksimal kapasitesi d ı ş ı nda, TUEP grubunda di ğ er parametrelerde olu mlu sonuç izlenmi ş tir
Abstract
bjective: To show change in residual urine volume, P detristol Q max and maximum bladder capacity in prostate symptom scores within early period six months after operation in prop ortion to pre-operati on in three group patients that were non-homogeneou s in terms of prostate volumes. Material and Methods: Seventy-five patients with LU SS were included in study, whose bladder outflow fully obstruction had been shown with pressure flow study. Fift y patients as healthy control were included. Of the 50, TVP was applied to 15, TUEP for 15, and TU RP for 20. Measurements of symptom scoring, Q max , P det Q max , maximum bladder capacity and residual urine volum e of all patients were performed pre-operation and post-operation. Paired T-test and Kruskal-Wallis test were used in statistical analysis. P value less than 0.05 was accepted as significant. Results: The mean age for TVP, TU EP and TURP groups we re 68.6, 61.3 and 66.6 years, respectively. Postoperative mean prostate volumes for TVP, TUEP and TURP groups were calculated as 79.1 g, 39.75 g and 54.8 g, respectively. Three gro ups were evaluated in terms of symptom scores, reduction in residual urine volume, P detrisol Q max , and maximum bladder capacity before a nd after operation. Significant changes were detected for these parameters of patients in the TVP and TURP. On the other hand, no significant change was monitored only in terms of maximum bladder capacity was monitored in patients in the TUEP group. However, significant change was monitored in symptom scores, reduction in residual urine volume, P detrisol Q max . Conclusion: While almost similar si gnificant result was monitored in t erms of prostate symptom scores, reduction in residual urine volume P detrisol Q max , and maximum bladder capacity in patient groups applied TVP and TURP despite of different prostate volume, positive result was also monitored in other parameters, except maximum bladder capacity in TUEP group, in selected patients.
