Özet

Amaç: Mikroinvaziv meme karsinomu (MİMK) Dünya Sağlık Örgütü tarafından 1mm’ye eşit veya daha küçük boyutta meme stromasını infiltre eden bir veya birden fazla mikroskopik odak ile karakterize lezyon olarak tanımlanmıştır. Bu çalışmada MİMK tanılı hastalarda klinik ve patolojik parametrelerin, rekürrens, uzak metastaz gelişimi ve prognoz üzerindeki etkilerinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Ocak 2006-Aralık 2014 tarihleri arasında meme kanseri tanısı konulan hastalar retrospektif olarak incelendi. MİMK tanısı konulan hastalar çalışmaya dahil edildi. Histopatolojik olarak invaziv meme karsinomunun tipi, nükleer derecesi, mikroinvaziv odak sayısı, lenf nodu metastazı varlığı, hormone reseptör durumu, insitu komponentin çapı, derecesi ve hormon reseptör durumu değerlendirildi. Ayrıca lokal rekürrens, uzak metastaz ve sağkalımlar ve histopatolojik parametrelerle ilişkisi değerlendirildi. Bulgular: MİMK tanılı 27 hasta çalışmaya dahil edildi. Bütün hastalar kadın cinsiyetteydi. Medyan tanı yaşı 56(25 - 78), medyan takip süresi 51(15-130)aydı. MİMK tanılı 27hastanın 25’inde klinik takip bilgilerine ulaşılabildi. Hastaların hiçbirisine neoadjuvan kemoterapi uygulanmadı. Hastaların 1/3’üne telle işaretleme ile eksizyon, 1/3’üne meme koruyucu cerrahi, 1/3’üne modifiye radikal mastektomi uygulandığı saptandı.. Olguların 13’ünde (%48,1) tek invaziv odak, 14’ünde (%51,9) birden çok invaziv odak mevcuttu. Duktal Karsinoma İn Situların medyan çapı 2,5(0,5-10)cm olarak hesaplandı. Yirmi hastanın hormon reseptör (östrojen ve progesteron) bilgilerine ulaşılabildi. Hormon reseptörleri 13 (%48,1) hastada pozitif, 7(%25,9) negatifti. Medyan 51 aylık izlemde 2(%8) hastada rekürrens meydana geldi,2(%8) hasta kaybedildi. Beş yıllık hastalıksız sağkalım%86.7, 5yıllık genel sağkalım %91.4 saptandı. Sonuç: MİMK tanılı olguların çoğunluğuna komedonekroz bulunan yüksek nükleer dereceli duktal karsinoma in situ eşlik etmekteydi. Mikroinvaziv odak sayısı, duktal karsinoma in situ çapı, nükleer derece ve hormon reseptör ekspresyonunun rekürrens ve sağkalımlar ile ilişkisi saptanmadı.

Abstract

Objective: The World Health Organisation defines microinvasive breast carcinoma (MIBC) as a lesion characterized by a single or multiple microscopic foci infiltrating the breast stroma, equal to 1 mm in size or smaller. The present study aims to investigate the impact of clinical and pathological parameters on the recurrence, metastasis, and prognosis in patients with the MIBC. Materials and Methods: A group of patients diagnosed with breast cancer from January2006 to December 2014 were examined retrospectively. Patients diagnosed with the MIBC were included in the study. A histopathological examination included the type of the invasive breast carcinoma, nuclear grade, number of microinvasive foci, presence of a lymph node metastasis, and hormone receptor status, diameter and grade and hormone receptor status of insitu component. In addition, local recurrence, distant metastasis, and survival characteristics and their relationship to the histopathological parameters were evaluated Results: Twenty-seven patients with the MIBC were included in the study. All the patients were female. Median age was 56 (25- 78). Median follow-up duration was 51(15-130) months. Among 27 cases of MIBC, clinical follow-up information was availiable in 25 cases. Neoadjuvant chemotherapy was not administered to any of the patients. It was determined that 1/3 of the patients had wire guided wide local excision, 1/3 of breast conserving surgery, 1/3 of modified radical mastectomy. There were only one invasive focus in 13 (48.1%) and multiple invasive focus in 14 (51.9%) patients. The median diamater of Ductal Carcinoma In Situ was 2.5 cm (0.5-10cm). Hormone receptor (estrogen and progesterone) information was available in 20 patients. Hormone receptors were positive in 13 (48.1%) patients and negative in 7 (25.9%) patients. Two (8%) patients developed recurrence and 2 (8%) patients died during the follow-up period. Five year disease free survival was 86.7% and 5 years overall survival was 91.4%. The number of microinvasive foci and ductal carcinoma in situ diamater and nuclear grade and hormone receptore expression were not related to recurrence and survival. Conclusion: In the majority of MIBC cases accompanied by a high nuclear grade Ductal Carcinoma In situ with comedo necrosis. The number of microinvasive foci and ductal carcinoma in situ diamater and nuclear grade and hormone receptore expression were not related to recurrence and survival.
