Tiroid Nodüllerinde Ultrason Rehberliğinde İkili Enjektör Modeli ile İnce İğne Aspirasyon Biyopsisi

Özet
Amaç: Retrospektif çalışmamızda üretmiş olduğumuz ikili enjektör modeli kullanılarak yapılan tiroid ince iğne aspirasyon biyopsi (İİAB )’sinin etkinliğini araştırmayı amaçladık. Gereç ve Yöntem: Şubat 2018-Mayıs 2018 tarihleri arasında, klinisyen istemiyle tarafımıza yönlendirilen tiroid nodülü olan hasta grubunda, bilgisayar ortamında tasarladığımız ve 3 boyutlu yazıcıdan çıktısını aldığımız ikili enjektör modeliyle USG klavuzluğunda aynı anda iki iğne ile 131 nodülden ince iğne aspirasyon biyopsisi yaptık. Sitolojik tanılar Bethesda sınıflaması baz alınarak yapılmış olup, olguların ultrasonografi bulguları belirlenen kriterler dahilinde kaydedilmiştir. Bulgular: Tirioid nodülü İİAB işlemi yapılan hastalarda yaş ortalaması 51,66±12,64 idi. Nodüllerin boyut ortalaması 21,83±5,4mm idi. Hasta yaş grupları ile biyopsi sonuçları arasında istatistiksel olarak anlamlı fark saptanmadı (p>0,05). Değerlendirmeye alınan olgularının %14,50 (19/131)’i yanlış, %85,50 (112/131)’u doğru bulunmuştur. İşleme bağlı komplikasyon her iki grupta da görülmemiştir. Sonuç: Ultrasonografi eşliğinde ince iğne aspirasyon biyopsisi etkili, güvenilir ve kolay uygulanabilir bir tanı yöntemidir. Doğru nodülden ve semisolid nodüllerde de doğru komponentten örnek alınmasında kullanılan yöntem ve teknik önemli yardımcılardır. Çalışmamızda ikili enjektör modeli kullanılarak yapılan tirod biyopsisi başarılı bir metod olarak bulunmuştur.
Abstract
Objective: In our retrospective study, ultrasound guided a novel dual injector model thyroid nodule fine needle aspiration biopsy (FNAB) as the measurement of success. Materials and Methods: A total of 131 thyroid nodules were applied with ultrasound guided fine needle aspiration biopsy with a novel dual injector model fine needle aspiration. This technique was designed in a computer environment and a 3D printed dual injector model was applied to the patient group between Feb 2018-May 2018 in our clinic. Cytological diagnoses were based on the Bethesda classification and ultrasonography findings of the cases were recorded within the specified criteria. Results: The mean age of the patients in the whole sample was 51,66±12,64 years. The mean size of the nodule was 21,83±5,4mm. No statistically significant difference was determined between the age and nodule size (p>0,05). The diagnosis was incorrect in 14,50% (19/131) and correct in 85.50 % (112/131) of cases. No technical complications were observed in patients. Conclusion: Ultrasound guided FNAB is an effective, reliable and easy-to-apply diagnostic method. The method and technique used to obtain samples from the correct component are important in the correct nodule and in semisolid nodules. The results of this study showed succesful method in the results of biopsies made using the binary injector model.
