Prostat Kanserinden Radyoterapi Alan Hastalarda Prostat Spesifik Antijen Takip Sonuçlarına Göre Tedavinin Değerlendirmesi

Özet

Amaç: Dünyada prostat kanseri insidansı her geçen gün hızla artmaktadır. Prostat kanseri tedavisinde primer hedef, prostat kanserinin tanı anındaki prognostik özelliklerinin bilinmesidir. Bunun için de rektal muayene bulguları, total prostat spesifik antijen ve serbest prostat spesifik antijen değerleri ile birlikte, transrektal biyopsi ve Gleason skoruna bakılması gerekmektedir. Bu nedenle çalışmada, hastaların tanı anındaki kan değerlerinin, radyoterapiden üç ay sonraki değerlerler ile karşılaştırılarak, tedaviye yanıt ve rme durumlarının değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmaya, Adana Numune Eğitin ve Araştırma Hastanesi Radyasyon Onkolojisine başvuran ve 2016 Mart-2017 Mayıs arası radyoterapi gören 27 hasta dahil edilmiştir. Hastaların radyoterapiye baş lamadan önce ve radyoterapi bittikten üç ay sonrasındaki; total prostat spesifik antijen ve serbest prostat spesifik antijen değerleri ile birlikte, karaciğer transaminazları, albumin, epidermal büyüme faktörü reseptörü, Na, K, üre, kreatinin ve tam kan değerlerine bakılmıştır. Bulgular: Rt öncesi ve sonrasi farklar bakimindan incelendiğinde; t2c grubunda tpsa, alt, lenfosit ve bazofil, t2b grubunda ise; tpsa, spsa, alt, egfr, üre, hb, htc, lenfosit ve nötrofil bakimindan rt öncesi ile sonrasi farklar istatistik olarak anlamli bulunmuştur (p<0.05). tpsa rt sonrasi, alp rt sonrasi, na rt öncesi, htc rt öncesi, lym rt öncesi ve sonrasi ile basofil rt sonrasi özellikleri için orta grade ile yüksek grade gruplari arasindaki fark istatistik olarak anlamli bulunmuştur (p<0.05). orta grade grubunda tpsa, spsa, alt, egfr, hb, htc, lym, nötrofil ve bazofil, yüksek grade grubunda ise; tpsa, spsa, alt, lym ve bazofil bakimindan rt öncesi ile sonrasi farklarin istatistik olarak anlamli olduğu görülür (p<0.05). Sonuç: Sonuç olarak, ölçümler arasında anlamlı farkların bulunmuş olması; radyoterapi gören hastalarda, hastanın üç aylık kontrollerinde total prostat spesifik antijen ve serbest prostat spesifik antijen değerlerinin, prostat kanserinde tedavinin seçiminde öneml i olduğunu göstermekte ve tedavi planlanırken bu durumun göz önüne alınması önerilmektedir.

Abstract

Objective: Prostate cancer incidence is increasing day by day in the world. The primary target on the cure of prostate cancer is to know prognostic features during diagnosis. For this, it is necessary to rectal examination. Total prostate specific antigen and free prostate specific antigen are together evaluated and it is also should be looked at the result of the transrectal biopsy. For that reason, ın this workshop it is aimed to evaluate and compare the patients’ blood rates. While diagnosing and the rates three months later after radiotherapy. Material and Method: 27 patients taking radiotherapy, consulting to the Adana Numune Training and Research Hospital Radiation oncology service, are added in this study between 2016 March and 2017 May. It is looked at the patients’ radiotherapy total and free prostate specific antigen, albumin, epidermal growth factor receptor, Na, K, urea, creatinine readings. before starting radiotherapy and three months after radiotherapy completed. Results: when it is examined the difference pre and post rt, at the group t2c tpsa, alt, lenfosit and bazofil, as to the group t2b; tpsa, spsa, alt, egfr, üre;hb, htc, lenfosit and notrofil, and the difference of pre and post rt was found statistically significant ,(p<0.05).post tpsa rt, post alp rt ,pre na rt,pre htc rt, pre lym rt and post lym rt, basofil rt for the post spesifications medium grade and high grade groups’ difference was found statisticaly significant (p<0.05). at the medium group grade tpsa, spsa, alt, egfr, hb, htc, lym, nötrofil and bazofil, at the high grade groups; tpsa, spsa, alt, lym and in terms of pre and post bazofil rt differences are seen statisticaly significant Conclision: As a result, finding the meaningful difference between the measurements, and at the patients, getting radiotheraphy, at the three months periodic controls of the patients Total prostate spesific antigen and prostate specific antigen rates are shown that it is important and it is suggested that this situation should be considered while the treatment is planning.
