Derin Venöz Trombozu Hastalarında Mekanik Trombektomi: Tek Merkezli Tecrübemiz

Özet

Amaç: Derin ven trombozu (DVT) Virchow triadı olarak bilinen endotel harabiyeti, staz ve hipe rkoagulabilite sebebiyle venöz sistemde tromboz gelişimi olup erken evrede tedavi edilmezse sekel bulgular ve ölüme sebep olabilmektedir. Bu makalede mekanik trombektomi ile tedavi ettiğimiz alt ekstremite DVT’li hastalarda başarı oranını etkileyen faktörleri ve bu konudaki tecrübelerimizi paylaşmayı amaçladık. Gereç ve Yöntem: Son bir yıl içinde girişimsel radyoloji kliniğimizde akut ve subakut dönemdeki alt ekstremite venöz trombozu olan toplam 13 hastaya vena cava filtresi takılması sonrası damar içine katater ile girilerek trombolitik kullanmadan trombüsün parçalanıp aspire edilmesini sağlayan mekanik cihazlar ile mekanik trombektomi yapılmıştır Bulgular: Ultrasonografi bulgularına göre akut DVT tanısı alan 3 ve subakut DVT tanısı alan 10 hasta olmak üzere toplam 13 hasta çalışmaya dahil edildi.Akut DVT olgularında mekanik trombektomi ile lümende tama yakın açılma sağlandı. Subakut DVT olgularında tek başına mekanik trombektomi ile lümende minimal açılma sağlandı. Bir ay sonraki kontrolde ise akut dönemde olanlardan tama yakın açılma devam ederken subakut dönemdekilerde belirgin lümen açıklığı izlenmemiştir. Sonuç: Trombektomi işlemi son zamanlarda uygulanan bir yöntem olduğundan dolayı uzun vadede ne gibi sonuçları olduğu hakkında yeterli bilgiye sahip değiliz. Ancak trombüs yükünü azalttığından ve medikal tedavinin etkinliğini arttırdığından dolayı damarın tekrardan tromboze olma ihtimalini azalttığı ve uzun vadede pozitif sonuçlar verdiği düşünülmektedir. Bu oran vena kava gibi çapı geniş ve akım oranı fazla olan damarlardan distale doğru gidildikçe azalmaktadır. Elde ettiğimiz verilere göre mekanik trombektomi erken dönemde yapıldığında güvenli ve etkili bir tedavi yöntemidir.

Abstract

Introduction: Deep venous thrombosis (DVT) is the coagulum formation due to stasis, endothelial damage and hypercoagulability, which is also called as Virchow triad. It may cause severe sequel and death if it is not treated in a timely manner. In this article, we aimed to share the factors that affect the success rate of DVT patients treated with mechanical thrombectomy and our experience on this subject. Materials and Methods: Within the last year, in our interventional radiology clinic, mechanical thrombectomy has been performed on 13 patients with acute and subacute lower extremity venous thrombosis with the aid of mechanical devices that allow the thrombus to be disintegrated and aspirated without using thrombolytics after inserting a vena cava filter. Results: According to ultrasonography findings 3 patients with acute DVT diagnosis and 10 patients with subacute DVT were included in this study. In the case of acute DVT, mechanical thrombectomy led to near total luminal patency. In subacute DVT patients, with mechanical thrombectomy partial luminal patency was achieved. In the follow-up period of one month, while complete patency was observed in the cases of acute DVT, no significant luminal patency were observed in the cases of subacute DVT. Conclusıon: We don’t have enough information about what the long-term results of thrombectomy is, because it is a procedure that has recently been put intopractice. However, since it reduces the burden of thrombus and increases the effectiveness of medical therapy, it is thought that it reduces the likelihood of re-thrombosis and gives positive results in the long term. This possibility decreases from the large diameter and high flow veins, like vena cava, to the caudal veins. According to our data, mechanical thrombectomy is a safe and effective treatment method when it is performed in early period.
