Özet

Amaç: Okluzal çürüğün başlangıç düzeyindeyken teşhis edilmesi koruyucu tedaviler veya minimal invaziv yaklaşımlar ile tedavi edilebilmesini sağlar. Bu nedenle günümüzde geleneksel çürük teşhis yöntemlerine (görsel, dokunmasal ve radyografik) alternatif olarak lazer floresans ve elektriksel sistemlerin kullanıldığı yeni çürük teşhis yöntemleri geliştirilmiştir. Bu çalışmanın amacı; başlangıç düzeyindeki okluzal çürüklerin teşhisinde lazer floresans (DIAGNOdent Pen, KaVo, Almanya) ve elektriksel yöntemin (ECM IV Lode, Groningen, Hollanda) doğruluğunu ve tekrarlanabilirliğini in vitro olarak karşılaştırmaktır. Gereç ve Yöntem: Çalışmamız periodontal veya ortodontik nedenle çekilmiş makroskobik olarak bozulmamış 80 premolar diş üzerinde yapıldı. Dişlerin okluzal yüzeyleri florür içermeyen proflaksi patı ve döner fırça kullanarak temizlendi. Dişlerin okluzal yüzeylerinden fotoğraflar alınarak değerlendirilecek nokta belirlendi. 2 deneyimli dişhekimi tarafından okluzal yüzeylerdeki belirlenen noktalar DIAGNOdent Pen ve ECM IV kullanılarak değerlendirildi. 1 hafta sonra aynı işlemler tekrarlandı. Bağımsız bir dişhekimi ve histoloji uzmanı tarafından kesit alınmış dişler değerlendirildi. Verilerin istatistiksel olarak değerlendirilmesinde ağırlıklı kappa skorları hesaplandı. Bulgular: Her iki teşhis yöntemi de histoloji ile iyi düzeyde uyuma sahip iken DIAGNOdent Pen‟in gözlemci içi (1.Gözlemci κw:0,751; 2.Gözlemci κw:0,711) ve gözlemciler arası tekrarlanabilirliğin (1.Gözlem κw:0,643; 2.Gözlem κw:0,619) ECM IV ün gözlemci içi (1.Gözlemci κw:0,635; 2. Gözlemci κw:0,681) ve gözlemciler arası tekrarlanabilirliğinden (1.Gözlem κw:0,632; 2.Gözlem κw:0,603) daha yüksek olduğu bulunmuştur. Sonuç: Lazer floresans ve elektriksel yöntemler okluzal çürük teşhisinde doğru ve tekrarlanabilir sonuçlar verebiliği için klinikte ve epidemiyolojik çalışmalarda geleneksel yöntemlere alternatif olarak kullanılabilir.

Abstract

Objective: There is an increase in the rate of occlusal caries when the prevalence of tooth caries is decreasing in most European countries. Diagnosis of early occlusal caries lesions allows treatment with preventive treatments or minimally invasive approaches. For this reason, new caries diagnostic methods using laser fluorescence and electrical systems have been developed as an alternative to conventional caries diagnosis methods (visual, tactile and radiographic). The aim of this study was to compare the validity and reproducibility of the laser fluorescence system (DIAGNOdent Pen, KaVo, Germany) and the electrical system (ECM IV Lode, Groningen, The Netherlands) in the diagnosis of initial occlusal caries lesions Material and Method: Our study was performed on 80 premolar teeth that were macroscopically extracted due to periodontal or orthodontic reasons. Two dental practitioners evaluated the points on the occlusal surfaces using DIAGNOdent Pen and ECM IV. The same procedure was repeated after 1 week. The teeth were examined by an independent dentist and histology specialist. Weighted kappa values were calculated statistically. Results: Both diagnostic methods demonstrated good compatibility with histology, whereas DIAGNOdent Pen's intra-observer reproducibility (observer 1 κw: 0,751; observer 2 κw: 0,711) and inter-observer reproducibility (observation 1 κw: 0,643; observation 2 κw: 0,619) was found to be higher than ECM IV„s intra - observers repeatability (Observer 1 κw: 0,635, observer 2 κw: 0,681) and inter-observer repeatability (Observation 1 κw: 0,632; Observation 2: κw: 0,603). Conclusion: Laser fluorescence and electrical system can be used as an alternative to conventional methods in clinical and epidemiological studies to give valid and reproducibly results in the diagnosis of occlusal carie s.
