Özet
Yemenin ve emmenin 3 gündür kesildigi sikayetiyle klinigimize getirilen 3 aylık disi buzagıda klinik muayeneyi izleyerek sondalama yapılmıs, ancak sondanın torakal özefagus’ta takıldıgı ve ilerlemedigi belirlenmistir. Direkt radyografide obstruksiyona neden olan cisim görülmemistir. Tekrar sondalanan ve su içme deneyi yapılan buzagıda yabancı cismin özefagusu tıkadıgı, geçisin olmadıgı anlasılmıstır. Olguda operasyona karar verilmis ve operasyon öncesi hazırlıgı izleyerek xylazin HCl ile sedasyon saglanmıstır. Entübe edilen olguya % 2 sevofloran ile genel anestezi yapılmıstır. Steril sartlarda toraksın sol tarafı hazırlanmıstır. 7. interkostal aralıktan torakotomi yapılmıstır. Özefagusa ulasıldıktan sonra yabancı cismin diaframın yaklasık 12 cm yakınında tıkanıklıga neden oldugu belirlenmistir. Teknigine uygun sekilde özefagotomi yapılmıs ve yabancı cismin seker pancarı parçası oldugu belirlenmistir. Özefagus çift kat dikisle kapatılmıstır. Toraks teknigine uygun biçimde kapatılmıs ve sualtı direni uygulanmıstır. ki gün hospitalize edilen olguda 7 gün antibiyoterapi uygulanmıstır. Herhangi bir komplikasyonla karsılasılmamıs, telefonla buzagı sahibinden 6 ay süreyle bilgi alınmıs ve herhangi bir komplikasyonun olusmadıgı anlasılmıstır. Sonuç olarak buzagılarda torakal özefagus obstruksiyonu olgularında torakotomi ile yapılan bu sagaltım sekli klinik yapan meslektaslarımıza önerilir bulunmustur.
Abstract
A 3-month-old female calf referred to our clinics with the complaint of cease of eating and sucking for 3 days. The calf was orally probed after clinical examination, however it was seen that the probe was stucked and not advanced within the thoracal oesopahagus. Indirect radiographic examination, no involvement of foreign body that cause obstruction was observed. The calf was probed again and water-drink test was carried out indicating no passage. It was decided that the foreign body plugged the oesophagus. Surgical intervention was considered and the animal was prepared for surgery. Sedation was achieved by xylazine HCl. General anaesthesia was maintained by 2% sevoflorane after entubation. Under sterile condition, the left thorax was prepared and draped. Thoracotomy was done between the 6th and 7th ribs. The foreign body was noted 12 cm to the diaphragma after reaching the oesaphagus. Following the routine oesophagotomy the foreign body was observed as a piece of sugar beet. Oesophagus was closed by double layer suture and under water dren was applied. The calf was hospitalized for 2 days and antibiotic was given for 7 days. No complication was observed postoperatively. For 6 months, the owner was phoned and he did not see any complication. In conclusion, it was found that the thoracotomy for the cases of thoracal oesophageal obstruction in calves may be suggested to the clinicians.
