Özet
Bu çalışma Ankara Üniversitesi Veteriner Fakültesi Ortopedi ve Travmatoloji Kliniği'ne kırık şikayeti ile getirilen, değişik tür, ırk, yaş ve cinsiyette, eklem içi ve ekleme yakın kırığı bulunan 34 kedi (% 65,38) ve 18 köpek (% 34,62) olmak üzere toplam 52 olgu üzerinde gerçekleştirildi. Yapılan muayeneler sonucunda 21 olguda femur (% 40,38), 21 olguda humérus (% 40,38), 7 olguda tibia (% 13,47) ve 3 olguda da radius distalinde (% 5,77) kırık olduğu tespit edildi. Olgularda kırık şekillerinin ayrımı, Salter-Harris (SH) sınıflandırma metodu kullanılarak yapıldı. 24 olguda SH-Tip I (% 46,16), 10 olguda SH-Tip II (% 19,23), 10 olguda SH-Tip III (% 19,23) ve 8 olguda da SH-Tip IV kırığı (% 15,38) belirlendi. Kırkdokuz olguda operatif (% 94,23), 3 olguda (% 5,77) konservatif sağaltım uygulandı. Olguların değişen kırık tip ve şekillerine göre; mini titanium plak ve vida siste-mi (26 olgu), çapraz pin ve se'rklaj (5 olgu), tek intra-medüller pin ve serklaj (6 olgu) xenogreft kemik pin ve serklaj (2 olgu), Rush pini ve serklaj (4 olgu), trans-artiküler tek pin (2 olgu), serklaj (4 olgu) ve koaptasyon bandaj (3 olgu) uygulamaları ile sağaltımları yapıldı. Olgular, postoperatif dönemde ikişer hafta arayla klinik ve radyolojik kontrollere tabi tutuldular. Bu muaye-nelerin sonuçlarının değerlendirilmesi Modifiye Hanover Veteriner Fakültesi Küçük Hayvan Kliniği Değerlendirme Skalasına göre yapıldı. Buna göre eks-tremite fonksiyonu bakımından 35 olguda çok iyi (% 67,31), 9 olguda iyi (% 17,31), 2 olguda tatminkar (% 3,85) ve 6 olguda da kötü (% 11,54) sonuç elde edildi. Karşılaşılan 7 adet komplikasyonun; 2 olguda ankiloz, 2 olguda malunion, 2 olguda pin migrasyonu ve l olguda da sinostoz olduğu belirlendi.
Abstract
This study was carried out on 34 eats (65.38%) and 18 dogs (34.62%) a total of 52 cases of different species, breed, age and sex which were brought to Ankara University, Faculty of Veterinary Medicine, Department of Orthopaedics and Traumatology, for intraarticular fracture occurance and for fractures placed near the joint as well. On postoperative two weeks intervals, cases were examined clinically and radiologically. According to these examinations, it was determined that in 21 cases femur (40.30%), in 21 cases humérus (40.38%), in 7 cases tibia (13.47%) and in 3 cases radius (5.77%) fractures had occured. Cases were classified by using the Salter-Harris (SH) classification method. In 24 cases SH-Type I (46.16%), in 10 cases SH-Type II (19.23%), in 10 cases SH-Type III (19.23%) and in 8 cases SH-Type IV (15.38%) fractures were reported. Operative treatment was performed in 49 cases (94.23%) where 3 cases were (5.77%) were treated conservatively. According to the different types and shapes of cases, mini titanium plate and screw system (26 eases), cross-pinning and cerclage (5 cases), single intramedullary pin and cerclage (6 cases), xenogreft bone pin and cerclage (2 cases), Rush pin and cerclage (4 cases), transarticular single pin (2 cases), only cerclage (4 cases) and coaptation bandaging (3 cases) were applied. Results of the examinations, which were made postoperatively, were evaluated according to the Modified Hanover Faculty of Veterinary Medicine, Small Animal Clinic Evaluation Scala. As for as extremity function is considered, 35 cases (67.31%) were evaluated as exellent, 9 cases as good (17.30%), 2 cases as fair (3.89%) and 6 cases as bad (11.54%). 7 complications which were observed ankylosis in 2 eases, malunion in 2 cases, pin migration in 2 cases and synostosis in one case.
