Köpekte bilateral pannus olgusunun lamellar total keratektomi ile sağaltımı

Özet
U.U. Veteriner Fakültesi Cerrahi Kliniğine iki yıldır sürekli gözyaşı akıntısı, gözlerinde kırmızı renkli kitleler bulunduğu ve görüşünde azalma olduğu şikayeti ile Amerikan Cocker ırklı, 5 yaşlı, erkek köpek getirildi. Yapılan klinik muayene sonucunda, bilateral keratitis superficialis chronica (pannus) tanısı konuldu. Ayrıca bu hastada, yine her iki gözde Gl. membrana nictitans superficialis'in hiperplazisi, ileri derecede konjunktivitis follikularis ve sol gözde keratosel saptandı. Operatif sağaltıma karar verildi ve hasta hospitalize edildi.Genel anestezi altında, önce sol göz operasyon için hazırlandı. Keratektomi için korneaya daha iyi yaklaşım sağlanması amacıyla, ilk aşamada, hiperplaziye uğramış Gl. membrana nictitans superficialis'in ekstirpasyonu gerçekleştirildi. Takiben bulbusa askı dikişleri uygulandı. Limbus üzerinde saat 6 hizasından başlanarak korneanın üst iki katını içerecek şekilde, tüm kornea boyunca dairesel bir keşi yapıldı. Sonra kornea makası ve pensi kullanılarak, bu üst iki katman, korneanın alttaki katmanlarından ayrıldı ve uzaklaştırıldı. Böylece, total keratektomi tamamlanmış oldu. Net bir şeffaflık ve pupillar refleksin iyi bir şekilde gözlenmesi üzerine işlem sona erdirildi. Operasyon süresince göze Cyclopentholate hidrochlorür (Sikloplejin gtt.®- Abdi İbrahim), Dextran 70 ve Benzalconium chloride (Tears naturale®- Alcon), Latanoprost (Xalatan gtt.®- Alcon), Ciprofloksasin hidrochlorür (Ciloxan gtt.®- Alcon), Dexamethasone alcohol (Maxidex gtt.®- Alcon) aralıklı olarak damlatıldı. 3. göz kapağının bulbar yüzünde bulunan böğürtlen tarzındaki folliküllerin büyük bir kısmı, G1. membrana nictitans superficialis'in ekstirpasyonu esnasında uzaklaştırılmış olup, geri kalanı kürete edilmek suretiyle ortadan kaldırıIdı. G1. membrana nictitans superficialis'in ekstirpasyonu sonucu oluşan konjunktival ensizyon hattı, 6/0 PDS ile sürekli dikiş uygulaması ile kapatıldı. Tarsorafi yapıldı. Sağ gözde de aynı işlemler yapıldı. Postoperatif dönemde hastanın uyanmasını takiben, parenteral analjezik ve antibiyotik uygulandı. Postoperatif 4. gün tarsorafi dikişleri uzaklaştırılarak her iki göz de açık bırakıldı ve aynı damlalara devam edildi. Subkonjunktival olarak üç gün aralıklarla, üç kez Dexamethasone (Dekort amp.®- Deva) ve hazırlanan Siklosporin damla (l mi. Sandimmun amp.+ 4 mi. Rafinerize ayçiçek yağı karı-şımı) 15 gün süreyle uygulandı. 15. günde yapılan indirekt oftalmoskopik muayenede, fundus net bir şekilde gözlendi. Sonrasında tamamen iyileşen hasta taburcu edildi, iki ay sonraki kontrolde her iki gözün de durumu-nun iyi olduğu, korneaların şeffaf, pürüzsüz, pupillar refleksin normal olduğu gözlendi. Altı ay sonra hasta sahibi ve hekiminden alınan bilgiden aynı durumun devam ettiği, nükse ilişkin herhangi bir bulgu olmadığı öğrenildi.
Abstract
A five years old, male, American Cocker was brought to Uludağ University Department of Veterinary Surgery Clinic with complaint of epiphora lasting for 2 years, red colored masses on eyes and vision loss. As a result of clinical examination; it's diagnosed bilaterally Chronic Superficial Keratitis (Pannus). Beyond this, bilateral Gl. membrana nictitans superficialis hyperplasia, severe conjunctivitis follicularis and keratocel on left eye were determined. Surgical treatment was decided an the patient was hospitalized.Under general anesthesia, first the left eye was prepared for operation. To closer approach to the cornea for the keratectomy, in first stage, the hyperplastic Gl. membrana nictitans superficialis was extirpated. Then suspender sutures were applied to the bulbus. Starting on the ventral edge (6 o'clock) of the limbus, including the upper two layers of the cornea, circular incision was made. By using cornea scissors and forceps these two layers were seperated from ther layers and then extirpated. In this way, lamellar total keratectomy was completed. As a total clearness and pupillar reflex were observed, the process was finished. During the operation, Cyclopentholate hidrochlorür (Sikloplejin gtt.®-Abdi Ibrahim), Dextran 70 ve Benzalconium chloride, (Tears naturale®-Alcon), Latanoprost (Xalatan gtt.®- Alcon), Ciprofloksasin hidrokloriir (Ciloxan gtt.®- Alcon), Dexamethasone alcohol (Maxidex gtt.®- Alcon), were dropped periodically to the eye. Mostly follicules which were present in bulbar face of third eyelid were remove from at that time Gl. membrana nictitans superficialis was extirpated. The rest of follicules were done away with by curettage. The incision line on the Gl. membrana nictitans superficialis was closed with 6/0 PDS using continuous suture. Tarsorraphy was performed. The process was reperformed on the other eye. Postoperative analgesic and antibiotics were injected. On the postoperative 4th day, tarsorraphy sutures in eyelids were taken, eyes were leave to uncovered and the same drugs were dropped. Subconjunctival Dexamethasone (Dekortamp. ®- Deva) injected three times for every 3 days and Cyclosporin gtt. (1 ml. Sandimmun amp.+ 4 ml. rafmated sunflower oil mixture) which was prepared were applied for 15 days. In indirect ophtalmoscopic examination that performing in 15 th days, fundus was observed clearly. Furthermore, the patient was entirely in good health and given to his owner. After two months when the patient was reexamined, the eyes were healthy and the cornea were transparent, smooth and the pupillar reflex was normal. After six months, from the patient owner and patient's doctor were obtain information, it was learnt that condition of patient was good and same disease didn't recur.
