Bir köpek ve kedide iyatrojenik hidronefroz

Özet
U.Ü. Veteriner Fakültesi Cerrahi Anabilim Dalı Küçük Hayvan Kliniği'ne 2 gündür iştahsız olduğu, bu süre içinde 4 kez kustuğu, 1 haftadır hareketlerinde azalma şikayeti olan 1 yaşlı, dişi bir Van kedisi ile uzun süredir iştahsızlık ve konstipasyon şikayeti bulunan 2.5 yaşlı, dişi bir Puanter ırkı köpek getirildi. Alınan anamnezde, kedinin 5 ay önce özel bir klinikte ovaryohisterektomi ameliyatı, köpeğin ise 14 ay önce sezaryen ile birleştirilen ovaryohisterektomi ameliyatı geçirdiği öğrenildi. Yapılan klinik, laboratuar, radyolojik ve ultrasonografik muayeneler sonucunda, her iki hastada da sol böbrekte hidronefroz tanısı konuldu. Ayrıca köpekte ultrasonografik bulgular hidroureteri de belirtiyordu. Her iki olguda da nefrektomi yapılmasına karar verildi. Genel anestezi sonrası kedide klasik nefrektomi girişi olan yan kesi tercih edildi. Köpekte ise, radyolojik bulgular koprastaz ileus'una işaret ettiği için enterotomi de yapılmasına karar verildiğinden median giriş seçildi. Köpekte hidronefroz nedeni olan, önceki ameliyata ait ureter ligasyonunda kullanılan 4 adet ipek iplik, orta ureter hizasında görüldü ve uzaklaştırıldı. Postoperatif 7. gün dikişleri alındı ve her iki olgunun intravenöz ürografıleri (İVP) yapılarak diğer böbreklerin fonksiyonunu tam olarak yaptığı saptandı. Hastaların uzun dönemde yapılan kontrollerinde, genel durumlarının iyi olduğu ve düzenli bir şekilde idrar yaptıkları anlaşıldı. Alınan böbreklerin histopatolojik muayene sonuçlarında, tanı olarak kedide dissemine fokal interstisyel nefritis ve glomeruluslarda dejeneratif değişikliklerle belirlenen hidronefroz, köpekte ise glomeruluslarda aynı değişiklerle birlikte kortikal divertiküllerde kanama ile belirlenen hidronefroz ve hidroureter tanısı konuldu. Radyolojik ve ultrasonografik muayenelerde, kedide ilgili böbreğe ve uretere ilişkin başka bir obstrüktif lezyon saptanmaması ve hastaların öncesinde özel kliniklerde ovaryohisterektomi ve sezaryen ile birleştirilen ovaryohisterektomi geçirmeleri nedeniyle, bu ameliyatın bir komplikasyonu olarak kedide İyatrojenik hidronefroz, köpekte buna ek olarak hidroureter şekillendiği anlaşılmıştır. Köpekte ise hem ultrasonografik muayene sonucu hem de ameliyat sırasında uretere konulan ligatürlerin görülmesi ile postoperatif bir hidronefroz ve hidroureter net olarak belirlenmiştir.
Abstract
A 1-year-old female Van cat with the complaint of anorexia for 2 days, vomiting 4 times during this period and weakness and a 2.5 years old female Pointer dog with the complaint of anorexia and constipation for a long period of time were brought to the Clinic of Small Animal Surgery, Uludağ University Faculty of Veterinary Medicine. Both animals had a history an ovario- hysterectomy operation 5 months and 14 months ago. According to clinical, laboratory, radiological and ultrasonographical examination hydronephrosis was diagnosed in the left kidneys of both animals. A hydroureter was detected ultrasonographically in the dog as well. Nephrectomy was decided for both patients. Under general anesthesia lateral incision was performed in the cat. As the radiological findings of the dog revealed an ileus due to coprostasis, a median incision was preferred for enterotomy. In the dog, ligation of ureter at the mid level was detected as the cause of hydronephrosis and the sutures were removed. Skin sutures of the operation wound were removed on the seventh day of after surgery and IVP was performed in both patients to control the function of the other kidneys. Follow-up of the postoperative period revealed regular urination and appetite in both patients. According to the histopathological findings disseminated focal interstitial nephritis and hydronephrosis with degenerative differentiations of the glomerulus were diagnosed in the cat and hydronephrosis and same alterations were diagnosed in the dog with additional hemorrhage in the cortical diverticula. It was concluded that hydronephrosis in the cat and hydronephrosis and hydroureter in the dog were both complications of the ovariohysterectomy and cesarian section operations reported in the histories of the patients, because during radiological and ultrasonog-raphical examinations no other obstructive lesions could be detected in the kidneys and ureter. This situation was more obvious in the dog because the ligatures could be detected on the ureter during ultrasonographical examination and intraoperatively.
