Egzersize bağlı akciğer kanaması (EIPH) olan atlarda klinik, tracheobronkoskopik, hematolojik bulgular ve koagulasyon profilinin değerlendirilmesi

Özet
Bu çalışmada egzersize bağlı akciğer kanaması (EIPH) olan atlarda klinik, tracheobronkoskopik, hematolojik bulgular ve koagulasyon profilinin değerlendirilmesi amaçlandı. Çalışmanın materyalini egzersize bağlı akciğer kanaması olan farklı cinsiyette, 3-5 yaşlı 20 adet (EIPH grubu) ve sağlıklı 10 adet (kontrol grubu) safkan İngiliz atı oluşturdu. Tüm atların klinik (vücut sıcaklığı, kalp frekansı, solunum frekansı, akciğer auskultasyonu ve perkusyonu) ve tracheobronkoskopik muayeneleri yapıldı. Her'iki gruptaki atlardan kan örnekleri alındı, hematolojik muayeneler (total lökosit, eritrosit sayısı, hemoglobin, hematokrit değer, trombosit ve formül lokosit) ve koagulasyon profili (prothrombin zamanı (PT), aktive edilmiş parsiyel thromboplastin zamanı (APTT), fibrinojen, antithrombin III) değerlendirildi. EIPH grubunu oluşturan atların tracheobronkoskopik muayenesinde 3 atta 1.derece, 9 atta 2. derece, 8 atta 3. derece akciğer kanaması tespit edildi. EIPH grubunda eritrosit (p<0:01) ye trombosit (p<0.05) sayısı ile hematokrit değerin ,(p<0.01) kontrol grubundan daha düşük olduğu belirlendi. PT, APTT* fibrinojen ve antithrombin ili değerlerinde her iki grup arasında bir farklılık saptanmadı Bu çalışmayla egzersize bağlı akciğer kanamasının tanısında tracheobronkoskopik muayenenin gerekli olduğu ve kanamanın koagulasyon bozukluluğu sonucu oluşmadığı belirlendi. Ayrıca, hastalığın etiyolojisini ortaya koymak için kapsamlı çalışmaların yapılması gerektiği sonucuna varıldı.
Abstract
Evaluation of clinical, tracheobronchoscopical, hematological findings and coagulation profile in horses with exercise induced pulmonary hemorrhage (EIPH) was aimed in this study. Twenty Thoroughbred horses with EIPH and 10 healthy Thoroughbred horses of both sexes and aged 3-5 years were used as materials. Clinical (rectal temperature, pulsation and respiration rate, pulmonary auscultation and percussion findings) and tracheobronchoscopical examinations were performed. The blood samples were collected from all horses in both groups and evaluated for hematological (white blood cell, red blood cell, hemoglobin, hematocrit, platelet and differentiated white blood cell count) and coagulation profile (prothrombine time (PT), activated partial thromboplastine time (APTT), fibrinogen and antithrombin III). Tracheobronchoscopical examination of horses in EIPH group showed that 3 horses had 1st degree, 9 horses had 2nd degree and 8 horses had 3rd degree EIPH. In EIPH group RBC (p<0.01), PLT (p<0.005) counts and PCV (p<0.01) were lower than control group. There was no significant difference between two groups in coagulation profile. By this study it was determined that tracheobronchoscopical examination is of a great value in the diagnosis of exercise induced pulmonary hemorrhage and bleeding is not due to coagulation disorders. Moreover, it was concluded that further studies are needed to establish the etiology of EIPH.
