Özet

Kısa etkili anestezik olan propofol anestezi indüksiyonunda ve idamesinde kullanılmaktadır, fakat analjezik etkisi zayıftır. Köpeklerde gerçeklestirilen bu çalısmada propofol ve ek olarak fentanil’in hemodinamik etkileri ve antioksidan özelliginin arastırılması amaçlanmıstır. Sekiz adet erkek sokak köpegi iki esit gruba ayrıldı. Tüm hayvanlara anesteziden 30 dakika önce atropin sülfat ve xylazine HCl uygulandı. Grup I de 6,5 mg/kg bolus fentanilin intravenöz yolla uygulanmasıyla indüksiyon saglandı. Grup II de 4,5 mg/kg propofol ve 3,5 mcg/kg fentanilin birlikte bolus intravenöz uygulanmasıyla indüksiyon gerçeklestirildi. Her iki grupta indüksiyonda entübasyon öncesi 90 mcg/kg atracurium intravenöz yolla uygulandı. Entübasyonu izleyerek köpekler anestezi cihazına baglandı. Her iki grupta anestezi idamesi iki saat süreyle 0,4 mg/kg/dakika dozda propofolün infüzyonuyla saglandı. Hemodinamik parametrelerden nabız frekansı, ortalama ve diastolik kan basıncı grup I de grup II le karsılastırıldıgında düsük bulundu. Ortalama arteriyel kan basıncı grup I ve II de sırayla 89±29 mmHg ve 100±19 mmHg olarak gözlendi (p 0.05). Sözü edilen bu parametreler ısıgında propofol ve fentanil kombinasyonunun daha iyi, etkili ve daha dengeli anestezi olusturdugu fakat ekstübasyon periyodunu uzattıgı sonucuna varıldı.

Abstract

Short acting intravenous anesthetic agent propofol could be used for the induction and maintanance of anesthesia but it has weak analgesic action. In this study on dogs, we wish to search the effect of propofol and additional fentanyl on hemodynamics and antioxidant features. Eight male mongrel dogs were divided into two equal groups. All the animals have been received atropin sulphate and xylazine HCl 30 minutes before the anesthesia. In group I, 6.5 mg/kg intravenous blous propofol was used for the induction. In group II, 4.5 mg/kg propofol and 3.5 mcg/kg fentanyl were used together for the induction. Both groups were received 90 mcg/kg atracurium at the induction before the entubation. They were connected to the ventilator following the entubation. Anesthesia maintanance was obtained by 0.4 mg/kg/min. propofol infusion for 2 hours. Hemodynamic parameters as heart beat, diastolic and mean arterial pressures were found low in group I compared with group II. Mean arterial pressure was observed 89±29 mmHg and 100±19 mmHg in group I and group II, respectively (p<0.05). Cardiac output was found decreased in both groups but this was not statistically significant. Both SOD and MDA levels were found decreased in group I and II. SOD levels were 221.5±72.8 and 196.8±87.4 in groups I and II (p>0.05). According with the above mentioned parameters, propofol and fentanyl combination could be reached a better and much balanced anesthesia but could be effect the extubation period.
