Özet

Amaç: Bu çalışmada, merkezimizde gerçekleştirilmiş olan canlı vericiden sağ lob karaciğer transplantasyonlarında perioperatif mortaliteye etki eden risk faktörleri incelenmiştir. Hastalar ve Yöntem: Merkezimizde gerçekleştirilen 200 adet canlı vericili sağ lob karaciğer nakli olgusu retrospektif olarak değerlendirildi. Perioperatif mortaliteye etki eden olası faktörler açısından yaş, cinsiyet, Meld skoru, BMI, UNOS skoru, preoperatif döneme ait total bilirubin, kreatinin, asit, asit enfeksiyonu, ensefalopati, özofagus varisi kanaması öyküsü, parsiyel portal ven trombozu, hepatosellüler karsinom tanısı, operatif faktörler olarak ise greft ağırlığının vücut ağırlığına oranı ve MHV alımı incelendi. Bulgular: İstatistik analiz sonrasında UNOS skorunun düşük olması, MELD skorunun yüksekliği, asit enfeksiyonu geçirmiş olmak mortalite ile istatistik olarak anlamlı düzeyde bağlantılı idi. Sonuç: Yoğun bakım tedavisine ihtiyacı olan veya siroz komplikasyonları gelişmiş hastalara yapılan karaciğer nakli sonrası perioperatif mortalite riskinin, optimum şartlarda opere edilen hastalara oranla daha yüksek olduğu sonucuna varılmıştır.

Abstract

Purpose: Our aim was to determine the risk factors affecting the perioperative mortality in right lobe living donor liver transplantation. Patients and Methods: Two hundred right lobe living donor liver transplantations were retrospectively evaluated in our center. The data consisted of patients' age, gender, BMI, MELD and UNOS scores, portal vein thrombosis, creatinin, bilirubin, ascites, ascites infection, bleeding from eosephagus varices, encephalopathy, hepatocellular carcinoma, graft-weight to body-weight ratio and MHV harvesting. Results: A higher MELD score, lower UNOS score and history of ascites infection were significantly correlated with perioperative mortality. Conclusion: Our results suggest that, the patients who require intensive care unit treatment before the transplantation and also have cirrhosis complications are associated with high perioperative mortality
