Özet

Amaç: Okült kisto-biliyer ilişki; karaciğer kist hidatiğinde sık karşılaşılan, genellikle postoperatif dönemde ortaya çıkan ve birtakım komplikasyonlara neden olan bir problemdir. Bu çalışmada peroperatif dönemde okült kisto-biliyer ilişkinin tespit edilmesinde metilen mavisi enjeksiyonunun etkinliğinin ortaya konulması amaçlanmaktadır. Hastalar ve Yöntem: Karaciğer kist hidatiği tanısı konulan ve konservatif cerrahi tedavi uygulanan 35 hasta çalışmaya alındı. Kist kavitesinde safra boyalı kist sıvısı olan ve belirgin kisto-biliyer ilişki bulunan hastalar tespit edildi. Geriye kalan hastalarda kist sıvısı safra boyalı olsun veya olmasın, okült kisto-biliyer ilişkinin teşhisi için ana safra kanalı klemplendi ve safra yollarına metilen mavisi enjeksiyonu yapıldı. Bulgular: Kist sıvısı safra boyalı olan 8 hastanın 5'inde (%63) belirgin kisto-biliyer ilişki tespit edildi. 3 hastada (%37) ise kist kavitesinde ısrarlı aramalara rağmen bulunamayan okült kisto-biliyer ilişki, metilen mavisi enjeksiyonu ile ortaya konuldu. Geriye kalan 27 hastanın 6'sında (%22) okült kisto-biliyer ilişki; kist sıvısının safra boyalı olmaması veya kisto-biliyer ilişkiyi düşündürecek herhangi bir bulgu olmamasına rağmen, uygulanan metilen mavisi enjeksiyonu ile gösterildi. Sonuç: Konservatif cerrahi tedavi uygulanan karaciğer kist hidatiği hastalarında metilen mavisi enjeksiyonu, peroperatif dönemde okült kisto-biliyer ilişkinin ortaya konulmasında etkili bir yöntem olarak kullanılabilir.

Abstract

Purpose: Occult cystobiliary fistula is frequently encountered in hydatic liver disease that is usually overt in the postoperative period and leads to certain complications. In the present study it is aimed to evaluate the efficacy of methylene blue injection technique in determining the occult cystobiliary communication in the peroperative period. Materials and Methods: Thirty five patients with the diagnosis of hydatic liver disease in whom conservative surgery has been performed were included in the study. The patients with biliary soiling of the cyst fluid and overt cystobiliary communication have been determined. In the remaining patient regardless of bile stain the common bile duct was isolated and methylene blue injection was performed. Results: Of the eight patients with bile stain of the cyst fluid, a cystobiliary communication was shown in 5 (63%). In the remaining 3 patients (37%) in whom no communication could be demonstrated, methylene blue injection showed the communication. In the remaining 27 patients with no sign of cystobiliary communication methylene blue injection showed a communication in 6 (22%). Conclusion: In hydatic liver disease in which conservative surgery has been performed methylene blue injection technique is a safe and effective procedure in the demonstration of occult cystobiliary communication
