Özet

Amaç: İnsizyonel fıtık ameliyatları sonrası görülen yara yeri infeksiyonu ile kanser, barsak açılması, ameliyatın acil-elektif oluşu, sentetik yama uygulaması ve dren kullanımı ilişkisini irdelemek. Hastalar ve Yöntemler: İnsizyonel fıtık nedeni ile son üç yılda (Ocak 2006-Aralık 2008) ameliyat edilen olgular çalışmaya alındı. Hastalar yaş, cinsiyet, ameliyatın acil veya elektif oluşu, fıtık defektinin boyutu, onarım şekli, dren kullanılıp kullanılmadığı ve kullanıldıysa sayısı, kanser varlığı, gastro intestinal sistemin açılıp açılmadığı, kullanılan sentetik yama türü, yara yeri infeksiyonu oluşumu ve infeksiyon oluşanlarda üreyen mikroorganizmalar yönünden incelendi. Bulgular: Ameliyat edilen 62 olgunun 18'inde yara yeri infeksiyonu gelişti. Barsak yaralanması olan 15 olgunun 14'ünde, kanseri bulunan 14 olgunun 10'unda, acil ameliyat edilen 12 olgunun 9'unda infeksiyon gelişti. Tek dren kullanılan 9 olguda infeksiyon gelişmezken, iki veya daha fazla dren kullanılan 53 olgunun 18' inde infeksiyon saptandı. Yatış süresi, infeksiyon oluşan grupta 31,4±22,4, oluşmayanlarda 9,9±11,5 gün idi (P<0.0001). Sonuç: Çalışmamızda insizyonel fıtıklarda, yara yeri infeksiyonu oluşmasında, gastrointestinal sistemin açılması, kanser varlığı ve ameliyatın acil koşullarda yapılması önemli risk faktörleri olarak görüldü.

Abstract

Purpose: To evaluate the relation of incisional surgical site infection developed after operations for incisional hernias with malignancy, opening of intestines, status of the operations as urgent or elective and use of prosthetic materials and drains. Patients and Methods: Patients operated for incisional hernia during the last 3 years are included in the study. Age, sex, status of the operations as urgent or elective, size of the hernia defect, type of the repair, usage and number of drains, presence of malignancy, opening of gastrointestinal tract, type of the mesh used, development of wound infection and microorganisms grown in the wound are all searched. Results: Wound infection occurred in 18 of the 62 cases operated. Infection was reported in 14 of the 15 cases with bowel injury, 10 of the 14 cases with malignancy and 9 of the 12 cases operated on emergency. In 9 cases in which a single drain was used no infection was observed whereas infection developed in 18 out of 53 cases with two or more drains. Conclusion: In this study opening of the bowel, presence of malignancy and operations on emergency were found as important risk factors for the development of wound infection with incisional hernia management.
