Özet

Amaç: Bezoara bağlı intestinal obstrüksiyon nedeniyle cerrahi tedavi uygulanan hastalardaki predispozan faktörleri, tanı ve tedavi yöntemlerini analiz etmek. Hastalar ve Yöntem: Bezoara bağlı barsak tıkanıklığı nedeniyle ameliyat edilmiş 24 hasta geriye dönük olarak değerlendirildi. Bulgular: Hastaların yaş ortalaması 59±10,48 yıl olarak bulundu. Kadın/erkek oranı eşitti (12/12). 17(%70.8) hastada geçirilmiş gastroduodenal ülser cerrahisi; 16 (%66.7) hastada diş problemleri ve çiğneme sorunu vardı. Geçmişteki ülser cerrahileri ile bezoar nedeniyle yapılan ameliyatlar arasındaki ortanca süre 12 yıldı. 8 hastada (%33.3) diyabetes mellitus saptandı. Ameliyat öncesi dönemde 19 hastaya Bilgisayarlı tomografi çekildi. Bilgisayarlı tomografi ile hastaların 18'ine (%94.7) bezoar tanısı doğru olarak kondu. 18 hastada bezoar, parmakla ezilerek çekuma ilerletildi. 6 hastada ise enterotomi ile çıkarıldı. Bezoarlar, 19 hastada (%79.2) incebarsakta, 4 hastada (% 16.7) midedeydi. 1 hastada (%4.1) mide ve ileum seviyesinde eşzamanlı olarak mevcuttu. Ölümle sonlanan olgu yoktu. Ameliyat sonrası ortalama yatış süresi enterotomi yapılanlarda anlamlı olarak yüksek bulundu (p<0,001). Enterotomi ile bezoar çıkarılan 6 hastadaki cerrahi alanda enfeksiyon oranı (%33.3), bezoarların ezilerek kolona ilerletildiği 18 hastaya oranla (%11.1) yüksek bulunmasına rağmen, sonuç istatistiksel olarak anlamlı bulunmadı (p=0,23). Sonuç: Gastroduodenal cerrahi öyküsü, diyabet ve çiğneme problemi olan ileus hastalarında, altta yatan nedenin bezoar olabileceği akılda tutulmalıdır. Bezoar tanısında bilgisayarlı tomografi oldukça duyarlıdır. Mümkün oldukça bezoar ezilerek çekuma ilerletilmelidir.

Abstract

Purpose: To evaluate the predisposing factors, diagnosis and treatment modalities in bezoar patients operated due to ileus. Patients and Methods: Twenty four patients operated due to bezoars were evaluated retrospectively. Results: Half of the patients were male. The most important reason for bezoar formation was ulcer surgery in 17 (70.8%) patients. The mean time between the first surgery and bezoar operation was 12 years. Plain graphies were obtained from all patients, with 75% gasfluid levels ratio. Nineteen patients underwent abdominal computer tomography prior to the operation. In 18 (94.7%) patients, computer tomography was successful for bezoar diagnosis. During the operation, bezoars were crushed by fingers and pushed forward to distal bowel segments in 18 (75%) patients, whereas bezoars were removed by enterotomy in the remaining 6 (25%) patients. The localization of the bezoars was in small intestine and in stomach mostly. Seven patients (66.7%) had serious dental problems. Eight patients (33.3%) were diabetic. The duration of hospital stay was significantly higher in enterotomy patients. Conclusion: In ileus patients with previous gastric surgery or mastication problem or diabetes mellitus, bezoar should be kept in mind as a predisposing factor. Moreover, computer tomography is a very efficient method in bezoar diagnosis. Lastly, if possible, crushing by fingers and pushing forward to the distal bowel segments should be preferred as the surgical procedure for bezoars.
